Abstract:Objective:To observe the curative effect of enema with modified Dachengqi tang in adjuvant treatment for acute respiratory distress syndrome(ARDS). Methods:A total of 80 cases of ARDS patients were divided into the observation group and the control group according to the random number table method,40 cases in each group. The control group was given routine nursing and western medicine, and the observation group was additionally given enema with modified Dachengqi tang based on the treatment of the control group. Before and after treatment, the indexes such as partial pressure of oxygen(PaO2), partial pressure of carbon dioxide(PaCO2), saturation of blood oxygen(SaO2), and the ratio of partial pressure of oxygen in arterial blood to fractional concentration of inspiratory oxygen(PaO2/FiO2) in arteries were compared between the two groups; mechanical ventilation time and hospitalization time in ICU were recorded. In the two groups, scores of Acute Physiology and Chronic Health Evaluation Ⅱ Scoring System(APACHE Ⅱ) and scores of gastrointestinal dysfunction before treatment were compared with those after treatment; the levels of procalcitonin(PCT) and C- reactive protein(CRP) before and after treatment were detected;the incidence of complications of gastrointestinal dysfunction was recorded. Results: After treatment, the levels of PaO2, SaO2 and PaO2/FiO2 in the two groups were increased when compared with those before treatment, and the PaCO2 levels were decreased;the levels of PaO2,SaO2 and PaO2/FiO2 in the observation group were higher than those in the control group, and the PaCO2 level was lower than that in the control group, differences being significant(P<0.05). After treatment, the APACHEⅡ scores and scores of gastrointestinal dysfunction in the two groups were decreased when compared with those before treatment;the two scores in the observation group were lower than those in the control group(P<0.05). The mechanical ventilation time and hospitalization time in ICU in the observation group were shorter than those in the control group(P<0.05). After treatment, the levels of PCT and CRP in the two groups were lower than those before treatment(P<0.05);the two levels in the observation group were lower than those in the control group(P<0.05). The incidence of complications of gastrointestinal dysfunction was 35.0% in the observation group, lower than that of 87.5% in the control group(χ2=23.224,P<0.05). Conclusion:The therapy of enema with modified Dachengqi tang in adjuvant treatment for ARDS patients, can improve the ratio of partial pressure of oxygen in arterial blood to fractional concentration of inspiratory oxygen as well as the functions of pulmonary ventilation and pulmonary gas exchange, alleviate inflammatory responses and the degree of the disease, protect gastrointestinal function, shorten mechanical ventilation time, and promote recovery of respiratory function, which is worthy of clinical application.
