    目的:结合子午流注理论中气血在经络脏腑间的分布规律,分析2型糖尿病各时辰血糖情况,探讨各脏腑与糖尿病血糖控制之间的关系。方法:回顾性分析进行动态血糖监测的2型糖尿病患者386例,将测得的所有血糖数据按12时辰分别统计计算平均血糖(MBG)、评估血糖波动[血糖标准差(SDBG)]、低血糖指数(LBGI)、高血糖指数(HBGI),并分析各个时辰血糖情况与总体血糖状况的相关性。结果:各时辰MBG以巳时(脾) 最高,酉时(肾) 次之;各时辰MBG与全天总体血糖之间的相关性:亥时(三焦) 相关性最强,寅时(肺) 次之。各时辰血糖波动SDBG以酉时(肾) 最高,申时(膀胱) 次之;血糖波动SDBG相关性分析中亥时(三焦)、未时(小肠) 最强,寅时(肺) 次之。LBGI风险最高点在子时(胆),与总体情况相关性最强的在未时(小肠);HBGI 最高点在巳时(脾),与总体情况相关性最强在亥时(三焦)。结论:MBG 及HBGI 最高点均在巳时(脾),酉时(肾) 血糖波动最大,寅时(肺) 在MBG、血糖波动、HBGI中均有重要作用,与全天总体情况相关性分析,MBG、血糖波动、HBGI相关性最强的时辰均为亥时(三焦),LBGI最高点亦在亥时(三焦)。

    Abstract:

    Abstract:Objective:Based on the distribution rules of qi and blood in meridian and collateral as well as Zang-Fu in the theory of Ziwu Liuzhu, to analyze the blood glucose in patients with type 2 diabetes mellitus in different hours, and to discuss the relationship between the control of blood glucose and Zang-Fu. Methods:Retrospective analysis was conducted in 386 cases of patients who had type 2 diabetes mellitus and were under continuous glucose monitoring. The data of blood glucose were used for statistical calculation of mean blood glucose(MBG), standard deviation of blood glucose(SDBG) for blood glucose fluctuation, low blood glucose index(LBGI) and high blood glucose index(HBGI) in different hours. The relevance of blood glucose in different hours and that in general was analyzed. Results:The MBG peaked in Si Shi (9 am— 11 am), the period of the spleen, followed by You Shi (5 pm—7 pm), the period of the kidney. The highest relevance of MBG in different hours and the general blood glucose throughout a day was appeared in Hai Shi (9 pm—11 pm),the period of the triple energizers,followed by Yin Shi (3 am—5 am),the period of the lung. The highest SDBG was in You Shi,the period of the kidney,followed by Shen Shi (3 pm—5 pm),the period of the bladder. The strongest relevance of SDBG was in Hai Shi,the period of the triple energizers,and Wei Shi (1 pm—3 pm),the period of the small intestine,followed by Yin Shi, the period of the lung. In Hai Shi,the period of the triple energizers,LBGI was the highest;the strongest relevance of LBGI in different hours and that in general was in Wei Shi,the period of the small intestine. The HBGI reached the highest in Si Shi,the period of the spleen,and the strongest relevance of HBGI in different hours and that in general was in Hai Shi,the period of the triple energizers. Conclusion:The highest MBG and HBGI are in Si Shi,the period of the spleen. The largest blood glucose fluctuation is in You Shi,the period of the kidney. The period of the lung,Yin Shi,is important to MBG,blood glucose fluctuation and HBGI. The period of the triple energizers,Hai Shi is the period of the strongest relevance between MBG,blood glucose fluctuation as well as HBGI in different hours and those throughout a day respectively,and the period of the highest LBGI.
