    目的:观察不同血清 25-羟基维生素 D 水平儿童及青少年体质量、糖脂代谢指标及中心动脉压的变化情况。方法:选取 2017 年 8 月—2019 年 1 月的 800 例儿童及青少年作为研究对象,根据血清 25-羟基维生素 D 水平的不同将其分为 A 组(正常组)270 例、B 组(不足组)265 例及 C 组(缺乏组)265 例。比较 3 组肥胖率、糖代谢指标[空腹血糖(FBG)、糖化血红蛋白 (HbA1c) 及 C 肽 (C-P)]、脂代谢指标 [三酰甘油 (TG)、低密度脂蛋白 (LDL) 及高密度脂蛋白 (HDL)] 及中心动脉压,同时比较 C 组中不同缺乏程度 (<25 nmol/L 及≥25 nmol/L) 者的检测结果。结果:C 组 HDL (1.35±0.12) mmol/L 低于 A组与 B 组,肥胖率 17.74%、糖代谢指标 [FBG (4.87±0.45) mmol/L,HbA1c (4.98±0.72)%,C-P (1.40±0.28) μg/L]、其他脂代谢指标 [TG (1.78±0.31) mmol/L,LDL (2.40±0.31) mmol/L] 及中心动脉压 (113.63±5.63) mm Hg (1 mm Hg≈0.133 kPa) 均高于 A 组与 B 组,差异均有统计学意义 (P<0.05);且 C 组中不同缺乏程度者的肥胖率、中心动脉压、糖代谢指标及脂代谢指标比较,差异均有统计学意义(P<0.05)。结论:25-羟基维生素 D 缺乏者的 HDL 更低,糖代谢指标、其他脂代谢指标及中心动脉压均更高,25-羟基维生素 D 的缺乏可能与儿童及青少年肥胖、糖脂代谢异常有关,并对血压等存在一定影响,维持正常维生素 D 水平可能获益。

    Abstract:

    Abstract:Objective:To observe the changes in weight,indexes of glucose and lipid metabolism,and central aortic pressure in children and adolescents with different levels of 25-hydroxyvitamin D in serum. Methods:A total of 800 cases of children and adolescents were selected as the study objects,and divided into three groups according to different levels of 25- hydroxyvitamin D in serum,namely Group A(the normal group with 270 cases),Group B(the insufficiency group with 265 cases),and Group C( the deficiency group with 265 cases). The obesity rate,indexes of glucose metabolism including fasting blood glucose(FBG), glycosylated hemoglobin A1c(HbA1c), and C- peptide(C- P), indexes of lipid metabolism, namely triglyceride(TG),low density lipoprotein(LDL),and high density lipoprotein(HDL),and central aortic pressure were compared among the three groups; the detection results were compared among children and adolescents at different degrees of deficiency(<25 nmol/L and ≥25 nmol/L) in Group C. Results:In Group C,the concentration of HDL was 1.35±0.12 mmol/L, lower than those in Group A and B;the obesity rate was 17.74%;indexes of glucose metabolism [FPG(4.87±0.45) mmol/L, HbA1c(4.98±0.72)%,and C-P(1.40±0.28) μg/L],other indexes of lipid metabolism [TG(1.78±0.31) mmol/L and LDL(2.40± 0.31) mmol/L], and central aortic pressure(113.63 ± 5.63) mm Hg were higher than those in Group A and Group B; differences were significant(P<0.05). When compared the obesity rate,central aortic pressure,and indexes of glucose and lipid metabolism among children and adolescents at different degrees of deficiency in Group C,there was significance in differences(P<0.05). Conclusion: Children and adolescents deficient in 25- hydroxyvitamin D has lower HDL, as well as higher weight,indexes of glucose and lipid metabolism,and central aortic pressure. The deficiency of 25-hydroxyvitamin D is probably relevant to obesity and abnormal metabolism of glucose and lipid in children and adolescents,which has certain effect on their blood pressure. For children and adolescents,it is possibly beneficial to maintain the normal level of vitamin D.
