Clinical Study on Injection of Leonurus Japonicus Combined with Carboprost Trometh⁃ amine in Preventing Postpartum Hemorrhage in High- Risk Parturients Undergoing Cesarean Section

目的:观察高危剖宫产产妇采用益母草注射液联合卡前列素氨丁三醇预防产后出血临床效果。方法:将80 例高危剖宫产产妇随机分为观察组和对照组,每组40 例。对照组产妇给予缩宫素注射液及卡前列素氨丁三醇治疗,观察组产妇在对照组基础上给予益母草注射液治疗。对比2 组产妇术中、产后2 h、产后24 h 出血量;对比2 组产妇剖宫产后宫缩持续时间、恶露持续时间、子宫底下降速度;对比2 组产妇产时、产后3 d D-二聚体和纤维蛋白原(FIB) 的水平差异,对比2 组产妇不良反应发生率。结果:2 组产妇术中出血量比较,差异无统计学意义(P>0.05)。观察组产妇产后2 h、产后24 h 出血量较对照组少,差异有统计学意义(P<0.05)。观察组产妇产后宫缩持续时间较对照组长,恶露持续时间较对照组短,子宫底下降速度较对照组快,差异有统计学意义(P<0.05)。2 组产妇产时D-二聚体、FIB 水平比较,差异无统计学意义(P>0.05)。产后3 d,观察组D-二聚体水平较对照组低,FIB 水平较对照组高,差异有统计学意义(P<0.05)。观察组不良反应发生率17.50%,对照组不良反应发生率22.50%,2 组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:益母草注射液与卡前列素氨丁三醇联合使用,可明显改善高危剖宫产产妇产后出血情况,加强子宫收缩能力,止血效果确切,安全性高。
Abstract:
Abstract: Objective: To observe the clinical effect of injection of leonurus japonicus combined with carboprost tromethamine which high-risk parturients undergoing cesarean section selected. Methods:A total of 80 cases of high-risk parturients undergoing cesarean section were randomly divided into the observation group and the control group,40 cases in each group. The control group was given oxytocin injection and carboprost tromethamine, and the observation group was additionally given injection of leonurus japonicus based on the treatment of the control group. The bleeding volume during surgery and at the 2nd and 24th hours after delivery was compared between the two groups;the duration of uterine contraction and lochia,and the descending speed of fundus of uterus after cesarean section were compared between the two groups; the differences of the levels of D- dimer and fibrinogen(FIB) during delivery and on the 3rd day after delivery were compared between the two groups;the incidence of adverse reactions was compared between the two groups. Results:There was no significant difference being found in the comparison of the bleeding volume during surgery between the two groups(P>0.05). The bleeding volume at the 2nd and 24th hours after delivery in the observation group was less than that in the control group, the difference being significant(P<0.05). After delivery, the duration of uterine contraction in the observation group was longer than that in the control group,the duration of lochia shorter,and the descending speed of fundus of uterus faster, the difference being significant(P<0.05). There was no significant difference being found in the comparison of the levels of D- dimer and FIB during delivery between the two groups(P>0.05). On the 3rd day after delivery,the D- dimer level in the observation group was lower than that in the control group,and the FIB level was higher,the difference being significant(P< 0.05). The incidence of adverse reactions was 17.50% in the observation group and 22.50% in the control group,and there was no significant difference being found in the comparison of the incidence between the two groups(P>0.05). Conclusion: The therapy of injection of leonurus japonicus combined with carboprost tromethamine can significantly improve postpartum hemorrhage in high- risk parturients undergoing cesarean section, and strengthen uterine contraction ability, with definite hemostatic effect and high safety.
