Clinical Study on Shenmai Injection Combined with Gastric Perfusion with Radix Aconiti Lateralis Praeparata Improving Heart Function of Patients with Septic Shock
Abstract: Objective: To observe the improvement effect of Shenmai injection combined with gastric perfusion with Radix Aconiti Lateralis Praeparata on the heart function of patients with septic shock of yang-qi depletion type. Methods:A total of 72 cases of patients with septic shock of yang-qi depletion type were randomly divided into the control group and the observation group,36 cases in each group. Both groups were given fluid resuscitation,antibiotics,vasoactive medicine, and artificial respirator for assisting ventilation. During the first six hours in ICU,the experiment group was additionally given gastric perfusion with decoction of Radix Aconiti Lateralis Praeparata,and the control group was additionally given gastric perfusion with warm water. Indexes of pulse-indicated continuous cardiac output(PiCCO) through transpulmonary thermodilution technique [cardiac output(CO),cardiac index(CI),left ventricular ejection fraction(EF),index of lung edema], indexes of bedside ultrasonic testing including CO, CI and EF, and biochemical index, namely, pro- B- type natriuretic peptide(Pro-BNP) before and after treatment were observed and compared in the two groups. During treatment,volume of fluid resuscitation,dosage of vasoactive medicine,duration of respirator use,and 28- day mortality were observed and compared between the two groups. Results:Before treatment,when compared indexes of PiCCO,indexes of bedside ultrasound,and level of Pro-BNP between the two groups,there was no significance in differences(P>0.05);on the 12th, 24th,48th and 72th hours after treatment,each index above in the two groups was significantly improved when compared with that before treatment(P<0.05),and the improvement of each index above in the observation group was more significant than that in the control group on the 24th,48th and 72th hours after treatment(P<0.05). During treatment,when compared volume of fluid resuscitation and 28- day mortality, there was no significance in the difference(P>0.05); dosage of vasoactive medicine in the observation group was less than that in the control group,and duration of respirator use was significantly shorter, differences being significant(P<0.05). Conclusion: In the treatment of patients with septic shock of yang- qi depletion type,the therapy of Shenmai injection combined with gastric perfusion with Radix Aconiti Lateralis Praeparata can significantly increase ventricular ejection fraction,cardiac output,and the heart index,and significantly decrease level of Pro-BNP,so as to improve the heart function and promote rehabilitation.
