Exploration of Central Effect Mechanism of Visual Triple Needling at Lianquan and Tianrong for Dysphagia Caused by Cricopharyngeal Dysfunction Based on DTI Technology
Abstract:Objective:To observe the clinical effect of visual triple needling at Lianquan(CV 23) and Tianrong(SI 17) points for dysphagia caused by cricopharyngeal dysfunction(CD), and to explore its possible mechanism of the changes of the structure and density of white matter fiber tracts based on diffusion tensor imaging(DTI) technology. Methods: A total of 168 cases of patients with dysphagia caused by CD were divided into the observation group and the control group according to the random number table method, 84 cases in each group. The observation group was given visual triple needling at Lianquan(CV 23) and Tianrong(SI 17) points,and electroacupuncture was connected next to Jialianquan and Tianrong on each side;the control group was given routine acupuncture treatment. Both groups were treated for four weeks. Before and after treatment,the Videofluoroscopic Swallowing Study(VFSS) scores,the mobility of hyoid-larynx complex and the FA values of each cerebral area in the injured side in the two groups were compared. The clinical effect in the two groups was compared. Results:After treatment,the VFSS scores in the two groups were increased when compared with those before treatment(P<0.01),and the VFSS score in the observation group was higher than that in the control group(P<0.01). After treatment,the upward displacement and antelocation of hyoid bone as well as the upward displacement and antelocation of thyroid cartilage in the two groups were increased when compared with those before treatment(P<0.05); the upward displacement and antelocation of hyoid bone as well as the upward displacement and antelocation of thyroid cartilage in the observation group were larger than those in the control group(P<0.05). After treatment, the FA values of pedunculus cerebellaris inferior,pedunculus cerebellaris medius,pedunculus cerebellaris superior,pedunculus cerebri,corticopontine tract and corticonuclear tract in the injured side in the two groups were increased when compared with those before treatment(P<0.05),and the above FA values of each cerebral area in the injured side in the observation group were larger than those in the control group(P<0.05). The total effective rate was 97.6% in the observation group, higher than that of 89.3% in the control group,the difference being significant(P<0.05). Conclusion:The visual triple needling at Lianquan and Tianrong points for dysphagia caused by CD can improve the clinical symptoms of patients,whose mechanism may be the changes of the structure and density of related white matter fiber tracts hence strengthening its neural plasticity.
