    目的:观察清开灵注射液联合地西泮对失眠症患者睡眠质量及单胺类神经递质的影响。方法:选取158 例失眠症患者,按随机数字表法分为对照组及试验组,各79 例。对照组用地西泮治疗,试验组用地西泮联合清开灵注射液治疗,均治疗1 个月。观察2 组临床疗效、不良反应及治疗前后匹兹堡睡眠质量量表(PSQI) 评分、简易智能精神状态检查量表(MMSE) 评分、多导睡眠图参数(睡眠潜伏期、睡眠总时间、睡眠效率、醒觉时间)、5-羟吲哚乙酸(5-HIAA)、γ-氨基丁酸(GABA)、多巴胺(DA)、T 淋巴细胞亚群(CD4+/CD8+、CD4+、CD3+、CD8+) 水平。结果:试验组治疗总有效率为88.61%,高于对照组的75.95%(P<0.05)。与同组治疗前比较,治疗后2 组PSQI 评分及血清CD8+水平均降低(P<0.05),睡眠潜伏期、醒觉时间均缩短(P<0.05),睡眠总时间均延长(P<0.05),睡眠效率、MMSE 评分及血清5-HIAA、GABA、DA、CD4+、CD3+、CD4+/CD8+水平均升高(P<0.05);与对照组治疗后比较,试验组治疗后PSQI 评分及血清CD8+水平均较低(P<0.05),睡眠潜伏期、醒觉时间均较短(P<0.05),睡眠总时间较长(P<0.05),睡眠效率、MMSE 评分及血清5-HIAA、GABA、DA、CD4+、CD3+、CD4+/CD8+水平均较高(P<0.05)。2 组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:清开灵注射液联合地西泮治疗失眠症临床疗效显著,可显著调节单胺类神经递质及T 淋巴细胞亚群水平,进而有效改善睡眠质量,延长睡眠时间,促使患者认知功能提升,且安全性良好。

    Abstract:

    Abstract: Objective: To observe the effect of Qingkailing injection combined with diazepam on sleep quality and monoamine neurotransmitter of patients with insomnia. Methods: A total of 158 cases of patients with insomnia were selected and divided into the control group and the experiment group according to the random number table method , 79 cases in each group. The control group was treated with diazepam, and the experiment group was treated with Qingkailing injection combined with diazepam. Both groups were treated for one month. Clinical effect and adverse reactions were observed in the two groups. Before and after treatment,score of Pittsburgh Sleep Quality Index(PSQI),score of Mini- Mental State Examination(MMSE), polysomnography parameters(sleep onset latency, total sleep time, sleep efficiency, wake after sleep onset),as well as levels of 5- hydroxyindoleacetic acid(5- HIAA),γ- aminobutyric acid(GABA),dopamine (DA),T lymphocyte subsets(CD4+/CD8+,CD4+,CD3+,CD8+ ) levels were observed in the two groups. Results:The total effective rate was 88.61% in the experiment group, higher than that of 75.95% in the control group(P<0.05). When compared with those in the same group before treatment,PSQI scores and levels of serum CD8 + in the two groups were decreased after treatment(P<0.05), sleep onset latency and wake after sleep onset shortened(P<0.05), total sleep time prolonged(P<0.05),sleep efficiency as well as scores of MMSE and levels of 5- HIAA,GABA,DA,CD4 +,CD3 +,and CD4+/CD8+ in serum increased(P<0.05). After treatment, when compared with those in the control group, PSQI scores and levels of serum CD8 + were lower in the experiment group(P<0.05), sleep onset latency and wake after sleep onset shorter(P<0.05),total sleep time longer(P<0.05),sleep efficiency as well as MMSE scores and levels of 5-HIAA,GABA, DA, CD4 + , CD3 + , and CD4 +/CD8 + in serum higher(P<0.05). There was no significant difference being found in the comparison of incidence of adverse reactions between the two groups(P>0.05). Conclusion: The therapy of Qingkailing injection combined with diazepam has a significant clinical effect in the treatment of insomnia. It can significantly regulate levels of monoamine neurotransmitter and T lymphocyte subsets;thus,the sleep quality can be effectively improved,sleep time prolonged,and cognitive function enhanced,with good safety.
