夏枯草汤联合缬沙坦治疗原发性高血压病疗效观察 及对血管活性物质的影响

    目的:观察夏枯草汤联合缬沙坦治疗原发性高血压病的临床疗效及对血管活性物质的影响。方法:将原发性高血压病患者87 例随机分为对照组43 例和研究组44 例。对照组应用缬沙坦治疗,研究组应用缬沙坦联合夏枯草汤治疗。比较2 组24 h 动态血压检测水平、血脂变化、血管内皮功能及临床总体疗效。结果:研究组总有效率为97.7%,对照组为81.4%,2 组比较,差异有统计学意义(P<0.05)。治疗前,2 组日平均收缩压(dSBP)、日平均舒张压(dDBP)、夜平均收缩压(nSBP)、夜平均舒张压(nDBP)、24 h 平均收缩压(24 hSBP)、24 h 平均舒张压(24 hDBP) 水平比较,差异均无统计学意义(P>0.05);治疗后8 周、16 周,2 组dSBP、dDBP、nSBP、nDBP、24 hSBP、24 hDBP 水平均较治疗前降低(P<0.05),且研究组各项指标降低较对照组同时间段更显著(P<0.05)。治疗前,2 组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C) 水平比较,差异无统计学意义(P>0.05);治疗后,2 组TC、TG、LDL-C 水平均较治疗前降低(P<0.05)、HDL-C 水平较治疗前升高(P<0.05),且研究组各项指标改善较对照组更显著(P<0.05)。治疗前,2 组一氧化氮(NO)、内皮素1(ET-1)、内皮型一氧化氮合酶(eNOS)、前列环素(PGI-2) 水平比较,差异均无统计学意义(P>0.05);治疗后8 周、16 周,2 组NO、eNOS、PGI-2 水平均较治疗前升高(P<0.05)、ET-1 水平较治疗前降低(P<0.05),且研究组各项指标改善较对照组同时间段更显著(P<0.05)。结论:缬沙坦联合夏枯草汤治疗原发性高血压病疗效显著,可明显改善血脂水平,有效调节血管活性物质。

    Abstract:

    Abstract:Objectives:To observe the clinical effect of Xiakucao tang combined with valsartan on essential hypertension and its effect on vasoactive substances. Methods:A total of 87 cases of patients with essential hypertension were randomly divided into the control group and the study group,43 and 44 cases in each group respectively. The control group was treated with valsartan, and the study group was treated with valsartan combined with Xiakucao tang. The levels of 24 h dynamic monitoring of blood pressure,blood lipid changes,vascular endothelial function and overall clinical effects in the two groups were compared. Results: The total effective rate was 97.7% in the study group and 81.4% in the control group, the difference being significant(P<0.05). Before treatment,there was no significant difference being found in the comparisons of the mean levels of daily systolic blood pressure(dSBP), daily diastolic blood pressure(dDBP), night- time systolic blood pressure(nSBP), night- time mean diastolic blood pressure(nDBP), 24 h mean systolic blood pressure(24 hSBP) and 24 h mean diastolic blood pressure(24 hDBP) between the two groups(P>0.05). At 8 and 16 weeks after treatment, the mean levels of dSBP,dDBP,nSBP,nDBP,24 hSBP and 24 hDBP in the two groups were decreased when compared with those before treatment(P<0.05),and the decrease in the study group was more significant than that in the control group(P<0.05). Before treatment, there was no significant difference being found in the comparison of levels of total cholesterol(TC), triglycerides(TG), low- density lipoprotein- cholesterol(LDL- C) and high- density lipoprotein- cholesterol(HDL- C) between the two groups(P>0.05). After treatment,TC,TG and LDL-C in the two groups were decreased when compared with those before treatment(P<0.05), and HDL- C was increased(P<0.05); the improvement of each index in the study group was more significant than that in the control group(P<0.05). Before treatment,there was no significant difference being found in the comparison of levels of Nitric oxide(NO),endothelin- 1(ET- 1),endothelial nitric oxide synthase(eNOS) and prostacyclin (PGI-2) between the two groups(P>0.05). At 8 and 16 weeks after treatment,the levels of NO,eNOS and PGI-2 in the two groups were increased when compared with those before treatment(P<0.05),and level of ET- 1 was decreased(P<0.05); the improvement of each index in the study group was more significant than that in the control group(P<0.05). Conclusion: Xiakucao tang combined with valsartan for essential hypertension has a significant curative effect,and can improve blood lipid levels and effectively regulate vasoactive substances.
