    摘要:

    以胡希恕先生“以方类证”理论为基础,以苓桂术甘汤、五苓散、真武汤、泽泻汤4 首经方辨治水饮眩晕为例结合临证医案,分析总结胡希恕先生运用此4 首经方的辨证要点与治案思路。经分析认为苓桂术甘汤、五苓散、真武汤、泽泻汤皆可治疗由于水饮所致的眩晕,临证需根据不同病机及方证选择相应经方:五苓散治太阳表虚,水饮内停;泽泻汤治心下停饮,上迫头面;真武汤治少阴太阴合病,阳虚水泛;苓桂术甘汤治中阳不运,痰饮水气内生。

    Abstract:

    Abstract:This paper analyzes and summaries the key points of syndrome differentiation and treatment ideas of Prof. HU Xishu using the four classical prescriptions which are Linggui Zhugan tang,Wuling powder,Zhenwu tang and Zexie tang, based on his theory of “differentiating syndromes through prescriptions” and combined with the clinical medical case of using the above four classical prescriptions to treat fluid retention- induced vertigo. After analysis,it can be concluded that Linggui Zhugan tang,Wuling powder,Zhenwu tang and Zexie tang all can be used to treat fluid retention-induced vertigo, but the corresponding classical prescriptions in clinical practice should be selected according to different pathogenesis and syndrome types shown in the these prescriptions: Wuling powder is used to treat exterior deficiency of greater yang and interior fluid retention;Zexie tang is used to treat fluid retention below the heart going upward into the head and the face; Zhenwu tang is used to treat combined diseases of lesser yin and greater yin and edema due to yang deficiency; Linggui Zhugan tang is used to treat the spleen yang failing in transportation and the internal generation of phlegm rheum and fluid retention.
