Abstract：Based on the "syndrome differentiation of cold and heat" in eight-principle syndrome differentiation in Chinese medicine， this paper analyzes the treatment of diabetic nephropathy from the perspective of heat based on the integrated theory of theoretical tracing， etiology， pathogenesis， pathogenic mechanism and staging syndrome differentiation. According to the analysis，deficiency of congenital endowment and weakness of five viscera，overeating of fat and sweet food and intemperate diets， disharmony of emotions， wrong medication， internal heat， and food accumulation and stagnation could make fire constrained in the internal body，leading to the damage to thin fluid and overconsumption of thick fluid as well as yin deficiency and dryness- heat， and finally causing diabetic nephropathy. The renal damage in the early， middle and late stages of diabetic nephropathy is closely related to internal heat. In the early stage， internal heat is an important factor in the development of diabetic nephropathy and renal pathological damage. In the middle stage，heat attacks downwards to the kidney and lodged in kidney collaterals，over-consuming qi and damaging essence，leading to deficiency of kidney collaterals and obstructing heat and qi stagnation. In the late stage，internal heat continues causing consumption and damage and forming lower abdominal masses，and newly-contracted pathogens continue to endanger and intermingled with former pathogens，leading to serious kidney damage.
