    摘要:

    目的:观察复方血栓通胶囊对早期糖尿病肾病患者肾功能、尿足细胞及Nephrin 蛋白的影响。方法:将180 例2 型糖尿病并早期肾病患者按随机数字表法分为3 组,各60 例。对照组予控制血糖、血脂、血压等西医基础治疗;缬沙坦组予西医基础治疗联合缬沙坦胶囊治疗;复方血栓通胶囊组予西医基础治疗联合复方血栓通胶囊治疗。记录各组患者病程、年龄、体质量指数(BMI) 及血压水平,检测各组患者治疗前后糖代谢指标、脂代谢指标、肾脏功能指标、尿足细胞数及Nephrin 蛋白水平。结果:最终纳入统计,对照组58 例,缬沙坦组58 例,复方血栓通胶囊组57 例。治疗后,各组患者空腹血糖(FBG)、餐后2 h血糖(P2hBG)、糖化血红蛋白(HbA1c)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、肌酐(Cr)、尿微量白蛋白肌酐比值(UACR)、尿足细胞及Nephrin 蛋白水平均低于同组治疗前(P<0.05);复方血栓通胶囊组治疗后UACR、尿足细胞及Nephrin 蛋白水平显著低于缬沙坦组及对照组(P<0.05)。结论:复方血栓通胶囊联合控制血糖、血脂、血压等基础治疗可有效改善2 型糖尿病并早期肾病患者肾功能,减少足细胞脱落及Nephrin 蛋白丢失,延缓肾病进展。

    Abstract:

    Abstract: Objective: To observe the effect of compound Xueshuantong capsules on the kidney function, urinary podocytes and Nephrin protein of patients with early diabetic nephropathy. Methods: A total of 180 cases of patients with type 2 diabetes mellitus complicated with early nephropathy were divided into three groups according to the random number table method,with 60 cases in each group. The control group was given basic western medicine treatment to control blood glucose,blood lipid and blood pressure;the valsartan group was additionally treated with valsartan capsules based on the treatment of the control group; the compound Xueshuantong capsules group was additionally given Compound Xueshuantong capsules based on the treatment of the control group. The course of disease,age,body mass index(BMI) and blood pressure were recorded. The indexes of glucose metabolism, lipid metabolism and kidney function, the number of urinary podocytes and the levels of Nephrin protein were detected before and after treatment. Results: Finally, 58 cases remained in the control group,58 cases in the valsartan group,and 57 cases in the compound Xueshuantong capsules group. After treatment,levels of fasting blood glucose(FBG),2-hour postprandial blood glucose(P2hBG),glycosylated hemoglobin (HbA1c),triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL- C),creatinine(Cr),urine albumin creatinine ratio(UACR),urinary podocytes and Nephrin protein in each group were lower than those in the same group before treatment(P<0.05). After treatment,levels of UACR,urinary podocytes and Nephrin protein in the compound Xueshuantong capsules group were significantly lower than those in the valsartan group and the control group(P<0.05). Conclusion: Compound Xueshuantong capsules combined with basic treatment such as controling blood glucose,blood lipid and blood pressure can effectively improve the kidney function of patients with type 2 diabetic mellitus complicated with early nephropathy,reduce the loss of podocytes and Nephrin protein,and delay the progression of nephropathy.
