化瘀通脉汤联合西药对高血压病患者内皮功能、氧化应激反应的影响

    目的:观察化瘀通脉汤联合西药对高血压病患者内皮功能、氧化应激反应的影响。方法:选取164 例高血压病患者,按随机数字表法分为对照组及治疗组各82 例。对照组使用西药治疗,治疗组在对照组基础上使用化瘀通脉汤治疗。比较2 组治疗前后血压、24 h 收缩压标准差(24 h SSD)、24 h 舒张压标准差(24 h DSD) 水平的变化,比较2 组治疗前后过氧化氢酶(CAT)、谷胱甘肽过氧化物酶(GSH-Px)、晚期氧化蛋白产物(AOPP)、白细胞介素-17 (IL-17)、同型半胱氨酸(Hcy)、血管紧张素Ⅱ (AngⅡ)、内皮素-1 (ET-1)、一氧化氮(NO)、内皮依赖性舒张功能(FMD)、非依赖性血管舒张功能(NMD) 水平的变化,观察2 组临床疗效。结果:治疗后,2 组收缩压、舒张压、24 h SSD、24 h DSD 水平均较治疗前下降,治疗组上述4 项水平均低于对照组,差异均有统计学意义(P<0.05)。治疗后,2 组CAT、GSH-Px 水平均较治疗前上升,AOPP 水平均较治疗前下降,差异均有统计学意义(P<0.05);治疗后,治疗组CAT、GSH-Px 水平均高于对照组,AOPP 水平低于对照组,差异均有统计学意义(P<0.05)。治疗后,2 组IL-17、Hcy 水平均较治疗前下降,治疗组IL-17、Hcy 水平均低于对照组,差异均有统计学意义(P<0.05)。治疗后,2 组AngⅡ、ET-1 水平均较治疗前下降,NO 水平均较治疗前上升,差异均有统计学意义(P<0.05);治疗后,治疗组AngⅡ、ET-1 水平低于对照组,NO 水平高于对照组,差异均有统计学意义(P<0.05)。治疗后,2 组FMD 水平、NMD 水平均较治疗前上升,治疗组上述2 项水平均高于对照组,差异均有统计学意义(P<0.05)。对照组临床疗效总有效率为82.93%,治疗组临床疗效总有效率为93.90%,2 组比较,差异有统计学意义(P<0.05)。结论:化瘀通脉汤联合西药对高血压病患者的血压水平有所改善,可提高临床疗效,减轻氧化应激损伤,改善血管内皮功能,缓解机体炎症反应。

    Abstract:

    Abstract: Objective: To observe the effect of the therapy of Huayu Tongmai tang combined with western medicine on endothelial function and oxidative stress in patients with hypertension. Methods: A total of 164 patients with hypertension were selected and divided into the control group and the treatment group according to the random number table method,with 82 cases in each group. The control group was treated with western medicine,and the treatment group was additionally treated with Huayu Tongmai tang based on the treatment of the control group. Before and after treatment, the changes of levels of blood pressure,24- hour systolic blood standard pressure(24 h SSD) and 24- hour diastolic blood pressure(24 h DSD) were compared between the two groups. The changes of levels of catalase(CAT), glutathione peroxidase(GSH- PX), advanced oxidation protein product(AOPP), interleukin- 17(IL- 17), homocysteine (Hcy), angiotensin II(AngII), endothelin- 1(ET- 1), nitric oxide(NO), flow- mediated vasodilatation(FMD) and nitroglycerin- mediated dilation(NMD) were compared between the two groups before and after treatment,and the clinical effects in the two groups were observed. Results:After treatment,the levels of systolic blood pressure,diastolic blood pressure,24-hour SSD and 24-hour DSD in both groups were decreased when compared with those before treatment,and the above four levels in the treatment group were lower than those in the control group, differences being significant(P<0.05). After treatment, the levels of CAT and GSH- Px in both groups were increased when compared with those before treatment, and the levels of AOPP were decreased, differences being significant(P<0.05); after treatment, the levels of CAT and GSH-Px in the treatment group were higher than those in the control group,and the level of AOPP was lower,differences being significant(P<0.05). After treatment,the levels of IL-17 and Hcy in the two groups were decreased when compared with those before treatment,and the above two levels in the treatment group were lower than those in the control group, differences being significant(P<0.05). After treatment,the levels of AngⅡ and ET-1 in both groups were decreased when compared with those before treatment, and the level of NO was increased, differences being significant(P<0.05); after treatment, the levels of Ang Ⅱ and ET- 1 in the treatment group were lower than those in the control group,and the level of NO was higher,differences being significant(P<0.05). After treatment,the levels of FMD and NMD in both groups were increased when compared with those before treatment, and the above two levels in the treatment group were higher than those in the control group, differences being significant(P<0.05). The total clinical effective rate was 82.93% in the control group and 93.90% in the treatment group, the difference being significant(P<0.05). Conclusion: The therapy of Huayu Tongmai tang combined with western medicine can improve the clinical effect on blood pressure in patients with hypertension, reduce the damage of oxidative stress, improve the vascular endothelial function, and alleviate the inflammatory responses.
