Abstract: Objective: To observe the clinical effect of the therapy of superfine acupotomy combined with joint loosening on hemiplegia shoulder pain after stroke. Methods: A total of 69 patients with hemiplegic shoulder pain after stroke were randomly divided into the acupotomy group, the loosening group and the combination group(superfine acupotomy combined with joint loosening), with 23 cases in each group. The acupotomy group was treated with superfine acupotomy,the loosening group was treated with joint loosening,and the combination group was treated with superfine acupotomy combined with joint loosening. Before treatment and two and four weeks after treatment,the joint active range of motion and Numeric Rating Scale(NRS) in the three groups were observed and compared respectively. Results: The seven indexes of shoulder joint active range of motion had time effect(P<0.01), which showed that all indexes in the three groups were gradually improved with the ongoing treatment. After two weeks of treatment,the anteflexion,abduction,external rotation and horizontal adduction in the acupotomy group and the combination group were better than those in the loosening group,differences being significant(P< 0.05); after two weeks of treatment, the anteflexion and horizontal adduction in the combination group were better than those in the acupotomy group,differences being significant(P<0.05). After four weeks of treatment,the anteflexion,abduction,external rotation and horizontal adduction in the combination group were compared with those in the acupotomy group and the loosening group respectively,differences being significant(P<0.05);the above indexes in the acupotomy group were compared with those in the loosening group respectively, differences being significant(P<0.05), which suggested that the combination group had the best curative effect, followed by the acupotomy group, and the loosening group had the least curative effect. After two and four weeks of treatment, the scores of NRS in the three groups were compared, the difference being significant(P<0.01). After two and four weeks of treatment, the combination group was compared with the acupotomy group,differences being significant(P<0.05);the combination group was compared with the loosening group,the difference being significant(P<0.05);the acupotomy group was compared with the loosening group,the difference being significant(P<0.05). The combination group was better than the acupotomy group and the loosening group. Conclusion:The therapy of superfine acupotomy and joint loosening has a good effect on hemiplegia shoulder pain,which is better than that of simple superfine acupotomy and joint loosening.
