Effect of Zhuangjin Xugu Decoction Combined with Functional Exercise on Postopera⁃ tive Rehabilitation in Senile Patients After Surgery of Femoral Intertrochanteric Frac⁃ ture
Abstract: Objective: To observe the effect of Zhuangjin Xugu Decoction combined with functional exercise on postoperative rehabilitation in senile patients after surgery of femoral intertrochanteric fracture (FIF). Methods:A total of 92 senile patients with FIF were selected and divided into the observation group and the control group according to the random number table method,with 46 cases in each group. Both groups were given proximal femoral nail antirotation (PFNA) for treatment. After surgery,both groups were treated with western medicine for anti-infection,anti-coagulation and anti-osteoporosis;the control group was additionally given functional exercise,and the observation group was additionally treated with Zhuangjin Xugu Decoction based on the treatment of the control group. Both groups were treated for 4 weeks after surgery. The affected limb swelling, pain, swelling regression time, healing time of fracture,free weight-bearing time,levels of serum bone alkaline phosphatase (BALP),tartrate resistant acid phosphatase-5b (TRACP5b),total procollagen typeⅠN-peptide (tPⅠNP) and bone morphogenetic protein-2 (BMP-2),and the Harris Hip scores were compared between the two groups. Results:Seven days after surgery,the maximum values of having limb swelling,swelling values and scores of Visual Analogue Scale (VAS) for pain in the two groups were decreased when compared with those one day after surgry (P<0.05),and the maximum value of affected limb swelling,swelling value and VAS score for pain in the observation group were lower than those in the control group (P<0.05). The swelling regression time,healing time of fracture and free weight-bearing time in the observation group were shorter than those in the control group (P<0.05). Four weeks after surgery,the levels of serum BALP,tPⅠNP and BMP-2 in both groups were increased when compared with those one day after surgery (P<0.05),and the levels of serum BALP,tPⅠNP and BMP-2 in the observation group were higher than those in the control group (P<0.05);the serum TRACP5b levels in the two groups were decreased when compared with those one day after surgery (P<0.05),and the serum TRACP5b level in the observation group was lower than that in the control group (P<0.05). Four weeks after surgery,the functional score and Harris Hip total score in the observation group were higher than those in the control group (P<0.05); there was no significant difference being found in the comparisons of scores of pain,malformation and range of motion between the two groups (P>0.05). Conclusion:Based on western medicine,Zhuangjin Xugu Decoction combined with functional exercise on senile patients after surgery of FIF can effectively mitigate the affected limb swelling and pain,improve the bone metabolism indexes,promote the symphysis and recovery of hip joint function.
