Abstract: Objective: To observe the clinical effect of Yishen Xiezhuo Decoction combined with western medicine for diabetic kidney disease (DKD) at stage Ⅲ. Methods: A total of 90 cases of DKD patients at stage Ⅲ with kidney deficiency and damp-turbidity syndrome were selected and divided into the control group and the observation group according to the random number table method,with 45 cases in each group. Both groups were given exercise, diet guidance, and oral administration of hypoglycemic agents or subcutaneous injection of insulin for treatment, the control group was additionally treated with Irbesartan Tablets combined with Calcium Dobesilate Tablets,and the observation group was additionally treated with Yishen Xiezhuo Decoction based on the treatment of the control group. Both groups were treated for 1 month. The traditional Chinese medicine(TCM) syndrome scores,kidney function indexes and calcium- phosphorus metabolism indexes before and after treatment,as well as the clinical effects were compared between the two groups. The incidence of adverse reactions was recorded. Results: After treatment,the total rate of clinical effect was 93.33% in the observation group,higher than that of 77.78% in the control group (P<0.05). The total syndrome scores of TCM in the two groups were decreased when compared with those before treatment (P<0.05),and the total syndrome score of TCM in the observation group was lower than that in the control group (P<0.05). The levels of blood urea nitrogen (BUN) and serum creatinine (SCr) in the two groups were decreased when compared with those before treatment (P<0.05), and estimated glomerular filtration rates (eGFR)in the two groups were increased when compared with those before treatment (P<0.05); the levels of BUN and SCr in the observation group were lower than those in the control group (P<0.05),and eGFR in the observation group was higher than that in the control group (P<0.05). The levels of intact parathyroid hormone (iPTH) and phosphorus ions in the two groups were decreased when compared with those before treatment (P<0.05),and the calcium levels in the two groups were increased when compared with those before treatment (P<0.05); the levels of iPTH and phosphorus ions in the observation group were lower than those in the control group (P<0.05), and calcium level in the observation group was higher than that in the control group (P<0.05). The incidence of adverse reactions was 6.67% in the observation group and 8.89% in the control group, there being no significance in the difference (P>0.05). Conclusion: Yishen Xiezhuo Decoction combined with western medicine for DKD at stage Ⅲ with kidney deficiency and damp- turbidity syndrome has a better clinical effect than that of simple western medicine,which can improve kidney function and calcium-phosphorus metabolism levels of patients more effectively,and relieve their clinical symptoms,with good safety.
