    摘要:

    目的:基于数据挖掘研究分析姚新苗教授治疗腰椎间盘突出症的用药规律。方法:收集2021 年 1 月至2022 年6 月期间姚新苗教授治疗腰椎间盘突出症的中药处方,运用Excel 进行频数分析,运用SPSS Clementine 11.1 软件进行关联规则分析和聚类分析。结果:共纳入771 张处方,使用药物199 种。频次≥ 100 的中医证型有肝肾亏虚型、瘀血停滞型、肝郁气滞型、湿热痹阻型、寒湿凝滞型;使用频次前10 位的药 物为怀牛膝、大枣、柴胡、生姜、黄芩、炒白芍、姜半夏、茯苓、续断、甘草。应用关联规则分析各证型支持 度靠前的药物组合分别为熟地黄-山药、当归-川芎-炒白芍、姜半夏-柴胡-黄芩、黄芩-柴胡、桂枝-防风-炒 白芍;对整体用药前30 位的高频药物进行聚类分析,得到3 类有效聚类组合,分别为六味地黄丸加减、柴胡 类方加减、当归芍药汤加减。结论:姚新苗教授治疗腰椎间盘突出症强调整体观念,用药注重补肾、疏肝、健 脾、活血。

    Abstract:

    Abstract: Objective: To analyze the medication rules of Professor YAO Xinmiao in the treatment of lumbar disc herniation based on data mining. Methods: The Chinese medicine prescriptions of Professor YAO Xinmiao for the treatment of lumbar disc herniation from January 2021 to June 2022 were collected for a frequency analysis by Excel and a association rule analysis and a cluster analysis by SPSS Clementine 11.1 software. Results: A total of 771 prescriptions were included, involving 199 types of Chinese medicinals. Traditional Chinese medicine syndrome types with a frequency greater than or equal to 100 times included the liver- kidney depletion type,the blood stasis and stagnation type,the liver constraint and qi stagnation type, damp-heat obstruction type, and cold-damp coagulation and stagnation type. The top 10 medicinals with the highest frequency of use were Achyranthis Bidentatae Radix from Henan,Jujubae Fructus,Bupleuri Radix,Zingiberis Rhizoma Recens,Scutellariae Radix,stir-fried Paeoniae Radix Alba, Pinelliae Rhizoma Praeparatum Cum Zingibere et Alumine,Poria,Dipsaci Radix,and Glycyrrhizae Radix et Rhizoma. Based on the association rules analysis,the medicinal combination with the highest support for each syndrome type included Rehmanniae Radix Praeparata-Dioscoreae Rhizoma , Angelicae Sinensis Radix-Chuanxiong Rhizoma-stir-fried Paeoniae Radix Alba,Pinelliae Rhizoma Praeparatum Cum Zingibere et Alumine- Bupleuri Radix- Scutellariae Radix , Scutellariae Radix- Bupleuri Radix , and Cinnamomi Ramulus-Saposhnikoviae Radix-stir-fried Paeoniae Radix Alba. Cluster analysis was conducted on the top 30 high- frequency medicinals in the overall medication, and three effective cluster combinations were obtained,namely modified Liuwei Dihuang Pills,modified Chaihu-associated prescriptions,and modified Danggui Shaoyao Decoction. Conclusion: In the treatment of lumbar disc herniation, Professor YAO Xinmiao emphasizes a holistic approach and focuses on supplementing the kidney, soothing the liver, fortifying the spleen,and invigorating blood.
