    目的:观察柴胡加龙骨牡蛎汤加减辅助治疗肝胃郁热型胃食管反流病的临床疗效。方法:采用随 机数字表法将96例肝胃郁热型胃食管反流病患者分为治疗组与对照组各48例。对照组接受艾司奥美拉唑和莫 沙必利治疗,治疗组在对照组基础上联合柴胡加龙骨牡蛎汤加减治疗,2组连续治疗8周。比较2组临床疗效 及治疗前后反流性疾病问卷(RDQ)、食管黏膜分级评分、中医证候积分、匹兹堡睡眠质量指数(PSQI) 评 分、健康状况量表(SF-36) 评分、食管动力学指标。结果:治疗后,治疗组总有效率89.58%,高于对照组 75.00% (P<0.05)。治疗后,2 组中医证候积分、RDQ 评分、食管黏膜分级评分、PSQI评分均较治疗前降 低(P<0.05),且治疗组上述指标低于对照组(P<0.05)。治疗后,2组食管下端括约肌静息压、远端收缩积 分、SF-36评分均较治疗前升高(P<0.05),且治疗组上述食管动力学指标及SF-36评分高于对照组(P< 0.05)。结论:柴胡加龙骨牡蛎汤加减辅助治疗肝胃郁热型胃食管反流病疗效确切,能够减轻患者消化道症状, 改善食管动力学,提升睡眠质量和生活质量。

    Abstract:

    Abstract:Objective:To observe the clinical effect of modified Chaihu Plus Longgu Muli Decoction in adjuvant treatment of gastroesophageal reflux disease of liver-stomach heat constraint type. Methods: A total of 96 cases of patients with gastroesophageal reflux disease of liver-stomach heat constraint type were divided into the treatment group and the control group according to the random number table method, with 48 cases in each group. The control group was treated with Esomeprazole and Mosapride, and the treatment group was additionally treated with modified Chaihu Plus Longgu Muli Decoction based on the treatment of the control group. Both groups were continuously treated for 8 weeks. The scores of the Reflux Disease Questionnaire (RDQ), grading of esophageal mucosa, traditional Chinese medicine (TCM) syndromes,Pittsburgh Sleep Quality Index (PSQI) Scale and the 36-Item Short Form Health Survey (SF-36), and esophageal dynamic indicators before and after treatment, as well as clinical effects were compared between the two groups. Results: After treatment, the total effective rate was 89.58% in the treatment group,higher than that of 75.00% in the control group (P<0.05). After treatment,TCM syndrome scores, RDQ scores,grading scores of esophageal mucosa,and PSQI scores in the two groups were decreased when compared with those before treatment (P<0.05),and the above scores in the treatment group were lower than those in the control group (P<0.05). Resting pressure in the lower esophageal sphincter,distal contractile integral,and SF-36 scores in the two groups were increased when compared with those before treatment (P<0.05), and the above esophageal dynamic indicators and SF-36 scores in the treatment group were higher than those in the control group (P<0.05). Conclusion:Modified Chaihu Plus Longgu Muli Decoction has a definite curative effect in the adjuvant treatment of gastroesophageal reflux disease of liverstomach heat constraint type, which can alleviate gastrointestinal symptoms in patients, improve esophageal dynamics,and enhance sleep and quality of life.
