Abstract： Objective： To observe the clinical effect of Shi's Zhutan Tongluo Decoction combined with Shi's characteristic manipulation on lumbar disc herniation (LDH) with phlegm-stasis obstructing the collaterals syndrome. Methods： A total of 84 cases of LDH patients with phlegm-stasis obstructing the collaterals syndrome were selected and divided into the treatment group and the control group according to the random number table method， with 42 cases in each group. One case was excluded from the treatment group，and ultimately 42 cases were included in the control group and 41 cases in the treatment group. The control group was treated with Shi's Zhutan Tongluo Decoction，and the treatment group was treated with Shi's characteristic manipulation based on the treatment of the control group. Both groups were treated for four weeks. The clinical effects， Visual Analogue Scale (VAS) scores， Japanese Orthopedic Association Assessment Treatment Score (JOA)，Oswestry Dysfunction Index (ODI) scores，and incidence of adverse events were compared between the two groups. Results：After four weeks of treatment，the total effective rate was 95.12% in the treatment group，higher than that of 80.95% in the control group (P＜ 0.05). After treatment for one week，two weeks，and four weeks，the VAS scores in the two groups were decreased when compared with those before treatment (P＜0.05)， and the VAS scores in the treatment group were lower than those in the control group during the same period (P＜0.05). After treatment for one week，two weeks，and four weeks，the JOA scores in the two groups were increased when compared with those before treatment (P＜0.05)，and the JOA scores in the treatment group were higher than those in the control group during the same period (P＜0.05). After treatment for one week，two weeks，and four weeks， the ODI scores in the two groups were decreased when compared with those before treatment (P＜0.05). After one week of treatment，there was no significant difference in ODI scores between the two groups (P＞0.05). After two and four weeks of treatment， the ODI scores in the treatment group were lower than those in the control group during the same period (P＜0.05). During treatment，the incidence of adverse reactions was 2.38% in the control group， and there were no adverse reactions in the treatment group，there being no significant difference between the two groups (P＞0.05). Conclusion：Shi's Zhutan Tongluo Decoction combined with Shi's characteristic manipulation is safe and effective in treating LDH with phlegm-stasis obstructing the collaterals syndrome， which can significantly alleviate pain and improve lumbar function.
