Abstract: Objective: To observe the kidney protective effect of enema with Dachengqi Decoction on sepsis patients. Methods: A total of 94 sepsis patients admitted to the Intensive Care Unit (ICU) of Shunde Hospital of Guangzhou University of Chinese Medicine from January 2022 to December 202 were prospectively selected and randomly divided into the Dachengqi Decoction group and the routine treatment group,with 47 patients in each group. Both groups received standardized diagnosis and treatment according to sepsis related guidelines. On this basis, the Dachengqi Decoction group was given enema with Dachengqi Decoction once a day,while the routine treatment group was given Glycerol Enema once a day.Both treatment courses lasted for three days.Using patient death or 28 days as the endpoint of the study, white blood cell count (WBC), C-reactive protein (CRP), procalcitonin (PCT), and serum creatinine (SCr) levels were recorded on the first, second, and third days of treatment for the two groups of patients. At the same time, acute kidney injury (AKI) grading, number of blood purification cases, mechanical ventilation time, ICU stay time, hospitalization time, and 28-day mortality rates were recorded for both groups of patients.Results:On the first day of treatment,there was no significant difference in the levels of WBC,CRP,and PCT between the two groups (P>0.05).On the second and third day of treatment,the levels of WBC,CRP,and PCT in the Dachengqi Decoction group were lower than those in the routine treatment group (P<0.05).On the first and second day of treatment,there was no significant difference in SCr levels between the two groups (P>0.05).On the third day of treatment, the SCr level in the Dachengqi Decoction group was lower than that in the routine treatment group (P<0.05). After treatment, there was a significant difference in AKI grading between the two groups (Z= -2.42, P=0.019). There was no significant difference in blood purification rates, mechanical ventilation time, hospitalization time, and 28-day mortality rates between the two groups (P>0.05). The ICU stay time in the Dachengqi Decoction group was shorter than that in the routine treatment group (P<0.05).Conclusion:Enema with Dachengqi Decoction can alleviate systemic inflammatory responses in sepsis patients,improve kidney function,and reduce ICU hospitalization time.
