Abstract：Objective：To observe the curative effect of Houpo Paiqi Mixture combined with conventional therapy on early neurogenic bowel dysfunction （NBD） after cervical spinal cord injury （CSCI）. Methods：A total of 90 cases of patients with NBD after CSCI admitted to Luoyang Orthopedic-Traumatological Hospital of Henan Province from December 2021 to December 2023 were selected and divided into the treatment group and the control group according to the random number table method， with 45 cases in each group. Both groups were given life support and conventional modern medical treatment，and the treatment group was additionally treated with Houpo Paiqi Mixture. Clinical effects were compared between the two groups； the changes in traditional Chinese medicine syndrome scores， abdominal distension scores， NBD scores， white blood cell count （WBC）， C-reactive protein （CRP）， procalcitonin （PCT）， and Acute Physiology and Chronic Health EvaluationⅡ （APACHEⅡ） scores before and after treatment were compared between the two groups； the mechanical ventilation rates during treatment and average duration of mechanical ventilation within one week were compared between the two groups. Results：The total effective rate of clinical effect was 97.8%（ 44/45） in the treatment group and 71.1%（ 32/45） in the control group，with a significant difference（ P＜ 0.05）. After treatment，traditional Chinese medicine syndrome scores，abdominal distension scores，and NBD scores in the two groups were decreased when compared with those before treatment （P＜0.05）， with the treatment group showing lower scores than the control group （P＜0.05）. The scores of CRP and APACHEⅡ in the two groups were decreased when compared with those before treatment （P＜0.05）， and the levels of WBC and PCT in the treatment group were reduced when compared with those before treatment （P＜0.05）； the treatment group had lower scores of WBC，CRP，PCT，and APACHEⅡ than the control group （P＜0.05）. After one week of treatment，the number of patients requiring mechanical ventilation in the treatment group was decreased when compared with that before treatment （P＜0.05）. The average duration of mechanical ventilation within one week in the treatment group was shorter than that in the control group，with a significant difference （P＜0.05）. After one week of treatment，no severe adverse reactions such as allergies or arrhythmia were observed in either group. Conclusion： Houpo Paiqi Mixture combined with conventional therapy has a good curative effect in treating NBD after CSCI， which can improve gastrointestinal function，inhibit inflammatory responses，and reduce the duration of mechanical ventilation.
