Clinical Study on Wenyang Sanhan Xiaobi External Washing Prescription Combined with Cilostazol for Diabetic Peripheral Neuropathy of Yang Deficiency and Congealing Cold Type
Abstract: Objective: To observe the clinical effect of Wenyang Sanhan Xiaobi External Washing Prescription combined with Cilostazol on diabetic peripheral neuropathy (DPN) of yang deficiency and congealing cold type. Methods: A total of 104 DPN patients of yang deficiency and congealing cold type admitted to Linhai Hospital of Traditional Chinese Medicine from January 202 to February 2024 were randomly divided into the study group and the control group,with 52 patients in each group,using a random number table method. The control group received oral treatment with Cilostazol, while the study group received foot bath treatment with Wenyang Sanhan Xiaobi External Washing Prescription on the basis of the control group. Both groups were treated for four weeks. Compared the clinical efficacy, traditional Chinese medicine(TCM) syndrome scores, Toronto Clinical Scoring System (TCSS) scores, nerve conduction velocity, lower limb arterial blood flow velocity and incidence of adverse reactions between the two groups. Results: The total effective rate was 9.88% (46/49) in the study group, which was higher than that of 79.59% (9/49) in the control group (P<0.05). The scores of TCM syndromes including numbness in the limbs,cold pain at the end of the limbs, aversion of coldness, soreness and coldness in the waist and knees, fatigue and weakness,and laziness in speech and TCSS scores in the two groups were lower than those before treatment (P<0.05), and the scores of TCM syndromes and TCSS in the study group were lower than those in the control group (P<0.05). The sensory nerve conduction velocity (SCV) and motor nerve conduction velocity (MCV) of common peroneal nerve, median nerve, and posterior tibial nerve, as well as the blood flow velocity of the bilateral popliteal and dorsal foot arteries in the two groups were all increased when compared with those before treatment (P<0.05). The SCV and MCV of the common peroneal nerve, median nerve, and posterior tibial nerve, as well as the blood flow velocity of the bilateral popliteal and dorsal foot arteries in the study group were faster than those in the control group (P<0.05). Conclusion: The combination of Wenyang Sanhan Xiaobi External Washing Prescription and Cilostazol can further improve the clinical symptoms of DPN patients of yang deficiency and congealing cold, accelerate nerve conduction velocity and lower limb arterial blood flow velocity,with good therapeutic effect and good safety.
