Clinical Study on Acupoint Application of Shaoyao Gancao Decoction Combined with Adenosylcobalamin for Diabetic Peripheral Neuropathy of Yin Deficiency and Blood Stasis Type
Abstract:Objective:To observe the effects of acupoint application of Shaoyao Gancao Decoction combined with adenosylcobalamin on hemorheology and peripheral nerve conduction velocity in patients with diabetic peripheral neuropathy (DPN) of yin deficiency and blood stasis type. Methods:A total of 82 patients with DPN of yin deficiency and blood stasis type admitted to the Taizhou Integrated Chinese and Western Medicine Hospital from November 2021 to November 2023 were enrolled and randomly divided into the observation group and the control group by random number table method, with 41 patients in each group. The observation group received acupoint application of Shaoyao Gancao Decoction combined with intramuscular injection of adenosylcobalamin, while the control group received intramuscular injection of adenosylcobalamin alone. The scores of traditional Chinese medicine syndromes, hemorheological indicators (including whole blood high-shear viscosity,fibrinogen,hematocrit,and platelet adhesion rate), and peripheral nerve conduction velocities [sensory nerve conduction velocity (SNCV) and motor nerve conduction velocity (MNCV) of the median and peroneal nerves] before and after treatment were compared between the two groups. The incidence of adverse reactions was also compared. Results:After treatment,the scores of traditional Chinese medicine syndromes such as limb numbness, leg and foot cramps, soreness and weakness of the waist and knees, vexing heat in the five sensation centers, and insomnia with profuse dreaming were decreased in both groups (P<0.05),and the scores in the observation group were lower than those in the control group (P<0.05). After treatment,the whole blood high-shear viscosity,fibrinogen,hematocrit,and platelet adhesion rate were decreased in both groups (P<0.05),and the values in the observation group were lower than those in the control group (P<0.05). After treatment,the SNCV and MNCV of the median and peroneal nerves were increased in both groups (P<0.05), and the values in the observation group were higher than those in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion:Acupoint application of Shaoyao Gancao Decoction combined with intramuscular injection of adenosylcobalamin is beneficial for improving traditional Chinese medicine symptoms such as fear of cold in the hands and feet in patients with DPN of yin deficiency and blood stasis type,improving hemorheology,and enhancing peripheral nerve conduction velocity.
