Clinical Study on Ankle Acupuncture in Preventing Deep Venous Thrombosis of Lower Limbs After Total Hip Arthroplasty in the Senior Based on Theory of Twelve Cutaneous Regions
Abstract: Objective: To observe the curative effect of ankle acupuncture on the prevention of deep venous thrombosis (DVT) of lower limbs after total hip arthroplasty in the senior based on the theory of twelve cutaneous regions. Methods:A total of 85 elderly patients who underwent unilateral total hip arthroplasty in Tongde Hospital of Zhejiang Province from March 2022 to February 2024 were selected and divided into the control group (42 cases) and the observation group (43 cases) according to the random number table method. The control group was given conventional DVT prevention therapy,and the observation group was additionally given ankle acupuncture based on the theory of twelve cutaneous regions and the conventional therapy. The changes in coagulation function indicators [activated partial thromboplastin time (APTT), prothrombin time (PT) and D-dimer], venous flow rate on the operative side, scores of Visual Analogue Scale (VAS) of pain on the operative side, and circumference difference between the affected limb and the healthy limb one day and seven days after surgery, and the DVT scores and DVT detection rates were compared between the two groups. Results:Seven days after surgery,the APTT and PT in the two groups were elevated when compared with those one day after surgery (P<0.05),and the D-dimer levels were reduced when compared with those one day after surgery (P<0.05);the APTT and PT in the observation group were higher than those in the control group (P<0.05),and the D-dimer level was lower than that in the control group (P<0.05). Seven days after surgery,the femoral vein flow velocity and popliteal vein flow velocity on the operative side in the two groups were higher than those one day after surgery (P<0.05), and the femoral vein flow velocity and popliteal vein flow velocity on the operative side in the observation group were higher than those in the control group (P<0.05). Seven days after surgery,the VAS scores of pain on the operative side and the difference of circumference between the affected limb and the healthy limb in the two groups were lower than those one day after operation (P<0.05);the VAS score of pain on the operative side and the difference of circumference between the affected limb and the healthy limb in the observation group were lower than those in the control group (P<0.05). Seven days after surgery,the DVT score on the operative side and DVT detection rate in the observation group were lower than those in the control group,differences being significant (P<0.05). Conclusion: The ankle acupuncture based on theory of twelve cutaneous regions can improve the coagulation function of elderly patients with total hip arthroplasty,accelerate the venous blood flow velocity of the affected limb,reduce the degree of pain and edema,and help to reduce the incidence of DVT in additional to conventional preventive therapy.
