Clinical Study on Treatment of Infertility in Obese Polycystic Ovary Syndrome with Tiaojing Cuyun Prescription Combined with Catgut Embedment in Acupoints
Abstract: Objective: To observe the clinical effect of Tiaojing Cuyun Prescription combined with catgut embedment in acupoints in the treatment of infertility in obese polycystic ovary syndrome (PCOS). Methods: According to the random number table method, 80 patients with obese PCOS infertility treated in the Department of Chinese Medicine Gynecology of Pingdingshan Maternal and Child Health Hospital from October 2020 to October 2023 were divided into a control group and a treatment group,with 40 cases in each group. Finally,38 cases in each group completed the study( two cases were excluded in each group). The control group was given Metformin Hydrochloride Sustained-Release Tablets combined with Ethinylestradiol and Cyproterone Acetate Tablets, and the treatment group was additionally treated with Tiaojing Cuyun Prescription and catgut embedment in acupoints based on the control group. After three treatment courses, the clinical effects, traditional Chinese medicine (TCM)syndrome scores, estrogen levels [testosterone (T), estradiol (E2), luteinizing hormone (LH), and follicle-stimulating hormone (FSH)], glucose and lipid metabolism indicators [total cholesterol (TC), triglyceride (TG), fasting insulin (FINS), fasting blood glucose (FBG),and insulin resistance index (HOMA-IR)],body mass index (BMI),number of follicles and ovarian volume were evaluated in the two groups. A six-month follow-up was conducted to count the ovulation rate and pregnancy rate in the two groups. Results: After treatment, the total effective rate in the treatment group was 92.11%( 35/38),and that in the control group was 73.68%( 28/38),with the difference between the two groups being significant (P<0.05). After treatment, the TCM syndrome scores in the two groups were lower than those before treatment (P<0.05), and the TCM syndrome scores in the treatment group were lower than those in the control group (P<0.05). After treatment,the levels of serum T,E2,and LH in the two groups were lower than those before treatment (P<0.05),and the FSH levels were higher than those before treatment (P<0.05). Moreover,the levels of serum T,E2,and LH in the treatment group were lower than those in the control group (P<0.05),and the FSH level was higher than that in the control group (P<0.05). After treatment,the levels of TC,TG,FINS,FBG,HOMA-IR, and BMI in the two groups were lower than those before treatment (P<0.05),and the above indicators in the treatment group were lower than those in the control group (P<0.05). After treatment,the number of follicles and ovarian volume in the two groups were lower than those before treatment (P<0.05),and the number of follicles in the treatment group was less than that in the control group (P<0.05), and the ovarian volume was smaller than that in the control group (P<0.05). During the six-month follow-up,the ovulation rate and pregnancy rate in the treatment group were 52.63% (26/38) and 23.68% (9/38), respectively, and those in the control group were 28.95% (17/38) and 5.26%( 2/38),respectively;the differences between the two groups were significant( P<0.05). Conclusion:On the basis of routine western medicine treatment,the combination of Tiaojing Cuyun Prescription and catgut embedment in acupoints in the treatment of infertility in obese PCOS has definite efficacy. It can alleviate TCM syndromes,effectively regulate estrogen and blood lipid levels,down-regulate insulin resistance and BMI,and reduce the number of follicles and ovarian volume,thereby improving pregnancy conditions.
