Study on Application of Combination Use of Umbilical Hot Compress with Ginger and Auricular Point Seed-Pressing for Abdominal Distension in Non-Invasive Ventilation Patients with Acute Exacerbations of Chronic Obstructive Pulmonary Disease of Phlegm-Damp Obstructing Lung Type
Abstract: Objective: To observe the application effect of the combination use of umbilical hot compress with ginger and auricular point seed-pressing on abdominal distension in non-invasive ventilation patients with acute exacerbations of chronic obstructive pulmonary disease (AECOPD) of phlegm-damp obstructing lung type. Methods: A total of 80 AECOPD patients who underwent non-invasive ventilation in Lishui Traditional Chinese Medicine Hospital Affiliated to Zhejiang Chinese Medical University from October 2022 to October 2023 were selected and divided into the study group and the control group according to the random number table method, with 40 cases in each group. The control group was treated with conventional non-invasive positive pressure ventilation, and the study group was additionally treated with umbilical hot compress with ginger combined with auricular point seed-pressing based on the treatment of the control group. Both groups were treated for two weeks. The clinical effects in the two groups were evaluated,and the scores of gastrointestinal symptoms,bowel sounds,abdominal circumference,the levels of arterial blood gas parameters and inflammatory markers and traditional Chinese medicine syndrome scores were compared between the two groups. Results:The total effective rate was 90.00% (36/40) in the study group,higher than that of 70.00% (28/40) in the control group (P<0.05). After treatment, the scores of Gastrointestinal Symptom Rating Scale( GSRS) and abdominal circumference in the two groups were reduced when compared with those before treatment, and the frequency of bowel sound was elevated when compared with that before treatment (P<0.05);the GSRS score and abdominal circumference in the study group were lower than those in the control group,and the frequency of bowel sound was superior to that in the control group (P<0.05). After treatment, the partial pressure of carbon dioxide (PaCO2) in the two groups was dwindled when compared with that before treatment,and the partial pressure of oxygen (PaO2) was up-regulated when compared with that before treatment (P<0.05);the PaCO2 in the study group was lower than that in the control group, and the PaO2 was higher than that in the control group (P<0.05). After treatment,the levels of interleukin-1β (IL-1β),tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in the two groups were decreased when compared with those before treatment (P<0.05),and the levels of IL-1β,TNF-α and IL-6 in the study group were lower than those in the control group (P<0.05). After treatment, the traditional Chinese medicine syndrome scores in the two groups were decreased when compared with those before treatment (P< 0.05), and the above scores in the study group were lower than those in the control group (P<0.05). Conclusion: The combination use of umbilical hot compress with ginger and auricular point seed-pressing has a great curative effect on the abdominal distension in non-invasive ventilation AECOPD patients of phlegm-damp obstructing lung type, which can improve the clinical symptoms, decrease the levels of inflammatory factors and improve the blood gas parameters.
