Abstract： Objective： To observe the clinical effect of Bushen Yangchao Soft Extract and acupuncture therapy combined with estrogen and progesterone treatment on diminished ovarian reserve （DOR） complicated with recurrent spontaneous abortion （RSA） of kidney deficiency type ， and its effects on pregnancy outcomes and ovarian function. Methods： A total of 100 cases of patients with DOR complicated with RSA of kidney deficiency type treated at Wenzhou TCM Hospital of Zhejiang Chinese Medical University from July 2022 to June 2023 were selected. They were divided into the control group and the trial group accoring to the random number table method，with 50 cases in each group. The control group received estrogen and progesterone treatment，and the trial group was additionally treated with Bushen Yangchao Soft Extract combined with acupuncture therapy in addition to the control group's treatment. One treatment course consisted of three menstrual cycles. If pregnancy was not achieved after the first course， the second course began one month later， continuing until pregnancy was achieved or up to two courses. Clinical effects and pregnancy outcomes （clinical pregnancy rate，ongoing pregnancy rate，live birth rate） were compared between the two groups. Before and after treatment， ovarian function [follicle-stimulating hormone （FSH） and anti-müllerian hormone （AMH）] and traditional Chinese medicine syndrome scores were compared between the two groups. Results： After treatment， the total effective rate in the trial group was 90.00% （45/50）， higher than 72.00% （36/50） in the control group（ P＜0.05）. Clinical pregnancy rates，ongoing pregnancy rates，and live birth rates in the trial group were higher than those in the control group （P＜0.05）. AMH levels in the two groups were increased when compared with those before treatment （P＜0.05），with higher level in the trial group than in the control group （P＜0.05）. FSH levels in the two groups were decreased when compared with those before treatment （P＜0.05），with lower level in the trial group than in the control group （P＜0.05）. Traditional Chinese medicine syndrome scores in the two groups were decreased when compared with those before treatment （P＜0.05），with lower score in the trial group than in the control group （P＜0.05）. Conclusion： Bushen Yangchao Soft Extract combined with acupuncture therapy and estrogen and progesterone treatment has a good curative effect in treating DOR complicated with RSA of kidney deficiency type. It can alleviate clinical symptoms，improve ovarian function，and increase clinical pregnancy rate，ongoing pregnancy rate， and live birth rate，with good safety.
