Abstract:Objective:To observe the clinical effect of wet compress of Quyu Shengji Prescription combined with western medicine in the treatment of diabetic foot ulcer (DFU). Methods:A total of 86 cases of patients with refractory DFU admitted to Zhoushan Hospital of Traditional Chinese Medicine from December 2021 to December 2023 were selected and divided into the control group and the observation group according to the random number table method, with 43 cases in each group. The control group was treated with conventional western medicine, and the observation group was treated with wet compress of Quyu Shengji Prescription on the basis of the control group. Both groups were treated for two months. The clinical effects in the two groups were compared. The changes of the 6-point Behavioral Rating Scale( BRS-6) score,ulcer area,transcutaneous oxygen pressure(TcPO2) on the dorsum of the foot,internal diameter,blood flow velocity,blood flow volume of the dorsalis pedis artery,microvessel density in granulation tissue of ulcer surface, optical density of vascular endothelial growth factor (VEGF), levels of serum amyloid A (SAA), Toll-like receptor 9 (TLR9),tumor necrosis factor α (TNF-α),and lipoprotein-associated phospholipase A2 (Lp- PLA2) were compared before and after treatment in the two groups. Results:The total effective rate of clinical effect in the observation group was 97.67%(42/43), and that in the control group was 81.40%(35/43), the difference being significant (P<0.05). After treatment, the BRS-6 scores and the ulcer area in the two groups were decreased compared with those before treatment,and the TcPO2 was increased compared with that before treatment,differences being significant (P<0.05);the BRS-6 score in the observation group was lower than that in the control group,the ulcer area was smaller than that in the control group, and the TcPO2 was higher than that in the control group, differences being statistically significant (P<0.05). After treatment, the internal diameter, blood flow velocity, blood flow volume of the dorsalis pedis artery in the two groups were increased compared with those before treatment (P<0.05), and the above three indicators in the observation group were superior to those in the control group (P<0.05). After treatment,the microvessel density and optical density of VEGF in the two groups were elevated compared with those before treatment (P<0.05), and the microvessel density and optical density of VEGF in the observation group were higher than those in the control group (P<0.05). After treatment,the levels of SAA,TLR9, TNF-α,and Lp-PLA2 in the two groups were decreased compared with those before treatment (P<0.05),and the levels of SAA,TLR9,TNF-α,and Lp-PLA2 in the observation group were lower than those in the control group (P< 0.05). Conclusion:The therapy of wet compress of Quyu Shengji Prescription combined with western medicine exerts certain clinical effect in the treatment of DFU, which can inhibit the inflammatory responses of the body, promote angiogenesis and wound healing,improve arterial hemodynamics and microcirculation,and alleviate pain.
