Abstract:Objective:To observe the curative effect of the combination use of acupuncture and Chinese medicine fumigation on neck-shoulder pain with lumbar-leg pain. Methods: Using prospective controlled trial, a total of 142 patients with neck-shoulder pain with lumbar-leg pain admitted to the Department of Orthopaedics of Rui'an Hospital of Traditional Chinese Medicine from January 2022 to December 2023 were selected as the study subjects and were divided into the treatment group and the control group according to the random number table method , with 71 cases in each group. With two cases being respectively exclude from the treatment group and the control group, finally 69 cases in each group completed the trial. The control group was treated with Chinese medicine fumigation,and the treatment group was treated with acupuncture combined with Chinese medicine fumigation. Both groups were treated for four weeks. The clinical effects and incidence of adverse reactions,and the changes of pain degree [Visual Analogue Scale (VAS)], frequency of pain attack, duration of pain, cervical spine function [Clinical Assessment Scale for Cervical Spondylosis (CASCS)], shoulder joint function [Constant-Murley (C-M) Score], lumbar spine function [scores of Japanese Orthopedic Association (JOA)], extremity motor function [scores of Fugl-Meyer Assessment Scale (FMA)], laboratory indexes [C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR)] before and after treatment were compared between the two groups. Results:The total effective rate was 94.20% (65/ 69) in the treatment group and 82.61% (57/69) in the control group, with a statistically significant difference (P< 0.05). After treatment,the VAS scores of the two groups were reduced when compared with those before treatment,the frequency of pain attack was down-regulated when compared with that before treatment, and the duration of each session of pain was shortened when compared with that before treatment,differences being significant (P<0.05);the VAS score of the treatment group was lower than that of the control group,the frequency of pain attack was less than that of the control group, and the duration of pain was shorter than that of the control group, with statistically significant differences (P<0.05). After treatment,the scores of C-M,CASCS,JOA and FMA (upper and lower limbs) in the two groups were elevated when compared with those before treatment (P<0.05), and the above four scores in the treatment group were higher than those in the control group (P<0.05). After treatment,the levels of serum CRP and ESR in the two groups were dwindled when compared with those before treatment (P<0.05),and the levels of serum CRP and ESR in the treatment group were lower than those in the control group (P<0.05). The incidence of adverse reactions was 7.25% (5/69) in the treatment group and 5.80% (4/69) in the control group,no statistically significant difference was found (P>0.05). Conclusion:The combination use of acupuncture and Chinese medicine fumigation has a significant curative effective on neck-shoulder pain with lumbar-leg pain,which can reduce the degree of pain in resting state,the frequency of pain attack and the duration of pain,improve the cervical spine function,shoulder joint function,lumbar spine function and extremity motor function,and lessen the inflammatory responses with great safety.
