Clinical Study on Continuous Perfusion with Yinhuang Lotion Combined with Intermi⁃ ttent Negative Pressure Drainage Technology in Treating Venous Leg Ulcers
Abstract: Objective: To observe the clinical efficacy of continuous perfusion with Yinhuang Lotion combined with intermittent negative pressure drainage technology in treating venous leg ulcers (VLU). Methods: A total of 72 VLU patients treated at Wenzhou TCM Hospital of Zhejiang Chinese Medical University from July 2022 to June 2024 were divided into the control group and the trial group (36 cases each) using the radom number table method. Both groups received routine care,health education and anti-infective therapy. The control group received additional 0.9% Sodium Chloride Solution combined with intermittent negative pressure drainage technology, while the trial group received continuous perfusion with Yinhuang Lotion combined with the same negative pressure drainage technology. Treatment duration was one week for both groups. Clinical efficacy was compared based on traditional Chinese medicine syndrome scores, inflammatory factors [interleukin-6 (IL-6), and C-reactive protein (CRP)], drainage tube blockage condition, wound conditions, and incidence of adverse reactions before and after treatment. Results: The trial group showed significantly higher total effectiveness (97.22%, 35/36) than the control group (83.33%, 30/ 36) (P<0.05). After one week of treatment, both groups demonstrated reduced wound exudate, improved wound coloration,decreased pain,and reduced swelling scores (P<0.05),with the trial group showing greater improvement in all four parameters (P<0.05). The IL-6 and CRP levels decreased significantly in both groups (P<0.05), with lower levels in the trial group (P<0.05). The trial group showed larger granulation tissue coverage area and smaller wound size compared to the control group (P<0.05). There was no significant difference in drainage tube blockage rates, bacterial culture conversion rates, or incidence of adverse reaction (P>0.05). Conclusion: Continuous perfusion with Yinhuang Lotion combined with intermittent negative pressure drainage technology demonstrates superior efficacy in treating VLU,effectively alleviating symptoms,reducing wound inflammation reactions,improving wound microenvironment,and promoting granulation tissue formation,with good safety profile.
