Abstract：Objective：To observe the clinical effect of the combination use of Xiaoyong Yukui Prescription and quadruple therapy on helicobacter pylori （Hp）-positive gastric ulcer with syndrome of toxic-heat accumulation in the stomach. Methods：A total of 106 patients with Hp-positive gastric ulcer with syndrome of toxic-heat accumulation in the stomach admitted to Songyang People's Hospital from March 2021 to March 2024 were selected and divided into the observation group and the control group according to the random number table method，with 53 cases in each group. The control group was treated with quadruple therapy， and the observation group was additionally treated with Xiaoyong Yukui Decoction based on the treatment in the control group. Both groups were treated for two weeks. The clinical effects and the eradication rate of Hp，and the traditional Chinese medicine syndrome scores of epigastric pain，nausea and vomiting， belching with acid regurgitation， epigastric upset and anorexia， ulcer healing， scores of gastric mucosal histology， and interleukin （IL）-35 mRNA and IL-37 mRNA on gastric mucosa before and after treatment were compared between the two groups. Results： After treatment， the total effective rate was 94.34% （50/53） in the observation group， which was higher than that of 75.47% （40/53） in the control group （P＜0.05）. The traditional Chinese medicine syndrome scores of epigastric pain， nausea and vomiting， belching and acid regurgitation， epigastric upset and anorexia in the two groups were lower than those before treatment （P＜0.05）. The traditional Chinese medicine syndrome scores of epigastric pain，nausea and vomiting，belching with acid regurgitation，epigastric upset and anorexia in the observation group were lower than those in the control group （P＜0.05）. The eradication rate of Hp in the observation group was higher than that in the control group （P＜0.05）. The hyperemia area around the ulcer， the area of thick moss at the bottom and the area of regenerated epithelium around the ulcer in the two groups were decreased when compared with those before treatment （P＜0.05），and the hyperemia area around the ulcer，the area of thick moss at the bottom and the area of regenerated epithelium around the ulcer in the observation group were smaller than those in the control group （P＜0.05）. The scores of gastric mucosal gland density， inflammatory cell infiltration and mucosal thickness in the two groups were reduced when compared with those before treatment （P＜0.05），and the above scores in the observation group were lower than those in the control group （P＜0.05）. IL-35 mRNA and IL-37 mRNA in the two groups were higher than those before treatment，and the above two levels in the observation group were higher than those in the control group （P＜0.05）. Conclusion：The combination use of Xiaoyong Yukui Prescription and quadruple therapy can effectively improve the clinical effect on Hp-positive gastric ulcer with syndrome of toxicheat accumulation in the stomach，which can enhance the eradication rate of Hp，alleviate symptoms，improve gastric mucosal tissue，and promote the healing of ulcer. Its mechanism may be related to the regulation of IL-35 mRNA and IL-37 mRNA.
