Clinical Study on Modified Buyang Huanwu Decoction Hot Compress Combined with Extracorporeal Shock Wave Therapy for Frozen Shoulder of Qi Deficiency and Blood Stasis Type
Abstract: Objective: To observe the clinical efficacy of modified Buyang Huanwu Decoction hot compress combined with extracorporeal shock wave therapy in the treatment of frozen shoulder of qi deficiency and blood stasis type,and its impact on inflammatory factors. Methods:According to the random number table method,78 patients with frozen shoulder of qi deficiency and blood stasis type treated at Wenzhou TCM Hospital of Zhejiang Chinese Medical University from August 2023 to August 2024 were divided into the observation group(39 cases)and the control group (39 cases). The control group received extracorporeal shock wave therapy, while the observation group received additional modified Buyang Huanwu Decoction hot compress based on the control group's treatment. Both groups were treated for four weeks. The clinical efficacy after four weeks of treatment was compared between the two groups,along with changes in pain severity [Visual Analogue Scale(VAS)score],shoulder joint function [Constant-Murley shoulder function score],quality of life [Short Form-36(SF-36)score],and laboratory values levels [interleukin-6(IL-6), tumor necrosis factor-α(TNF-α),and transforming growth factor-β1(TGF-β1)] before treatment and after four weeks of treatment. Results:After four weeks of treatment,the total effective rate in the observation group was 97.44% (38/39),significantly higher than 79.49%(31/39)in the control group(P<0.05). VAS scores decreased in both groups compared to those before treatment(P<0.05),with the observation group showing lower VAS scores than the control group(P<0.05). Constant-Murley shoulder function and SF-36 scores increased in both groups compared to those before treatment(P<0.05),and both scores were higher in the observation group than in the control group(P< 0.05). Serum levels of IL-6,TNF-α,and TGF-β1 decreased in both groups compared to those before treatment(P< 0.05),with lower levels observed in the observation group than in the control group(P<0.05). Conclusion:The combination of extracorporeal shock wave therapy with modified Buyang Huanwu Decoction hot compress in the treatment of frozen shoulder of qi deficiency and blood stasis type improves the clinical efficacy,enhances shoulder joint function and quality of life of patients,and reduces inflammatory responses.
