Abstract:The clinical characteristics of lumbar disc herniation in youngsters are mild pain and pronounced signs, which are easy to be confused with other spinal diseases. Therefore,the experience of Pingle bone-setting school in treating lumbar disc herniation in youngsters is summarized from the aspects of etiology and pathogenesis, characteristics of disease onset, therapeutic mechanism and application of bone-setting manipulation. The analysis suggests that the etiology and pathogenesis of the disease can be summarized as follows:“immature governor vessel spine with underdeveloped physique-vitality”“exposure to external pathogens and improper lifestyle practices”and “physical trauma from falls and improper work-rest balance”;guided by Pingle bone-setting school's core concept of “holistic syndrome differentiation and concurrent treatment of bones and soft tissues with balance as the guiding principle”,the following bone-setting techniques,including traction extension,tendon-targeted stretching,rapid traction-rotation repositioning and three-step traction-spring reduction of lower limbs are applied according to different stages(acute, recovery and rehabilitation)of the disease and the considerations in selection, key points and risk control of manipulation. These techniques have unique advantages in non-surgical treatment by relieving pain, correcting spinal deformity,and restoring spinal biomechanical balance.
