Clinical Study on the Treatment of Chronic Periodontitis of Kidney-Deficiency and Fire-Excess Type Using Bushen Guchi Pills Combined with Povidone Iodine
Abstract: Objective: To observe the clinical efficacy of Bushen Guchi Pills combined with Povidone Iodine in treating chronic periodontitis of kidney-deficiency and fire-excess type. Methods:A total of 86 patients with chronic periodontitis of kidney-deficiency and fire-excess type treated at Huzhou Branch of Hangzhou Stomatological Hospital from September 2022 to September 2024 were selected and divided into an observation group and a control group,with 43 cases in each group,using a random number table method. Both groups received basic treatment. The control group was treated with Povidone Iodine on the basis of basic treatment,while the observation group was additionally treated with Bushen Guchi Pills on the basis of the control group. Both groups were treated for 14 days. Clinical efficacy,time to symptom improvement, and incidence of adverse reactions, as well as changes in traditional Chinese medicine syndrome scores,periodontal status,and inflammatory factors before and after treatment were compared between the two groups. Results: The total effective rate in the observation group was 95.35%(41/43), higher than that in the control group at 79.07%(34/43)(P<0.05). The observation group showed shorter symptom improvement times for chewing pain, gum swelling, bad breath, and tooth loosening compared to the control group(P<0.05). Both groups exhibited reduced traditional Chinese medicine syndrome scores for chewing weakness, tooth loosening and displacement,gum swelling,tooth sensitivity,tinnitus,vexing heat in the chest,palms and soles, dry mouth and throat, and mental fatigue after treatment(P<0.05), with the observation group showing lower scores for all eight subitems of traditional Chinese medicine syndrome than the control group(P<0.05). After 7 days and 14 days of treatment, both groups showed reductions in plaque index(PLI), bleeding index(BI), and gingival index(GI) compared to before treatment(P<0.05), and the observation group had lower PLI, BI, and GI scores than the control group at the same time points(P<0.05). After 14 days of treatment,levels of interleukin(IL)-1β,IL-6, C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),and leptin decreased in both groups compared to before treatment(P<0.05),with the observation group exhibiting lower levels of these markers than the control group(P< 0.05). Meanwhile,IL-10 levels increased in both groups compared to before treatment(P<0.05),and the observation group had higher IL-10 levels than the control group(P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion: Bushen Guchi Pills combined with Povidone Iodine is effective in treating chronic periodontitis with kidney-deficiency and fire-excess syndrome. It can alleviate clinical symptoms,improve periodontal status and gingival bleeding,and reduce inflammatory reactions.
