    目的:观察电针治疗类风湿关节炎(RA) 的疗效和可能的作用机制。方法:将48只SD雄性大鼠 随机分为正常组、模型组、电针组及甲氨蝶呤组各12 只。除正常组外,其余各组大鼠经尾根部皮下注射 0.1 mL完全弗氏佐剂,以制备佐剂性关节炎大鼠。正常组及模型组大鼠每天灌胃2 mL 0.9%氯化钠溶液,共灌 胃21天;电针组选取肾俞、足三里、三阴交及昆仑穴电针,每次留针30 min,每天1次,共电针21天;甲氨 蝶呤组按2 mg/kg的剂量灌胃甲氨蝶呤混悬液,每周1次,共灌胃3周。干预结束后,采用动物Micro-CT检测 各组大鼠骨质破坏程度,肢体肿胀测量仪检测各组大鼠足体积,流式细胞仪检测辅助性T细胞(Th) 1/Th2, ELISA法检测血清中干扰素-γ(INF-γ) 与白细胞介素-4(IL-4) 含量。结果:干预结束后,与正常组比较, 模型组大鼠骨破坏明显,足体积增大(P<0.05),足肿胀评分、Th1/Th2和INF-γ含量均升高(P<0.05),体 质量和IL-4含量降低(P<0.05)。与模型组比较,电针组和甲氨蝶呤组大鼠骨破坏均明显改善,IL-4含量升 高(P<0.05),足体积减小(P<0.05),足肿胀评分、Th1/Th2和INF-γ含量均降低(P<0.05)。与甲氨蝶呤组 比较,电针组Th1/Th2、IL-4均升高(P<0.05)。结论:电针能改善RA大鼠骨质破坏,其作用机制可能是通过 调节Th1/Th2细胞比例,增加体质量和IL-4含量以恢复免疫稳态。

    Abstract:

    Abstract:Objective:To observe the efficacy and possible mechanism of electroacupuncture in the treatment of rheumatoid arthritis(RA). Methods: A total of 48 male SD rats were randomly divided into the normal group, the model group, the electroacupuncture group and the Methotrexate group, with 12 rats in each group. Except for the normal group,the remaining groups were subcutaneously injected with 0.1 mL complete Freund's adjuvant through the tail root to induce the adjuvant arthritis rat model. The normal group and the model group were gavaged with 2 mL 0.9% sodium chloride solution daily for 21 days;the electroacupuncture group was given daily electroacupuncture once at the selected acupoints of the Shenshu(BL 23),Zusanli(ST 36),Sanyinjiao(SP 6)and Kunlun(BL60)lasting for 30 min for 21 days. The Methotrexate group was given 2 mg/kg Methotrexate solution by gavage once a week, three times in total. After intervention,the bone destruction degree of rats in each group was detected by animal Micro-CT,the foot volume of rats in each group was detected by limb volume measuring instrument,the ratio of Th1 helper cell and Th2 helper cell was detected by flow cytometry,and the levels of INF-γ and interleukin-4(IL-4)in serum were detected by ELISA. Results:After the intervention,compared with the normal group,the bone destruction in the model group was more obvious,the foot volume was increased(P<0.05),the scores of foot swelling and Th1/Th2 and INF-γ level were up-regulated(P<0.05),the body mass and the IL-4 level were reduced(P<0.05). The bone destruction of rats in the electroacupuncture group and the Methotrexate group was significantly improved when compared with those in the model group,the IL-4 levels were increased(P<0.05),the foot volume was decreased(P<0.05),the scores of foot swelling,Th1/Th2,and INF-γ levels were downregulated(P<0.05). Compared with the Methotrexate group,Th1/Th2 and IL-4 level in the electroacupuncture group were increased(P<0.05). Conclusion: The electroacupuncture can improve the bone destruction of RA rats,whose mechanism might be to restore immune homeostasis by regulating the Th1/Th2 ratio,increasing the body mass and the IL-4 level.
