Abstract: Objective: To explore the correlation among C-reactive protein-albumin-lymphocyte (CALLY) index,C-reactive protein-triglyceride-glucose index(CTI)and diabetic nephropathy(DN)complicated with sarcopenia. Methods:The medical records of 120 DN complicated with sarcopenia patients and 60 DN patients treated in Wenling Hospital of Traditional Chinese Medicine from January 2021 to April 2025 were retrospectively analyzed,the CALLY index and CTI level were compared between the two groups,and the relationship of CALLY index and CTI level with step count, grip strength and appendicular skeletal muscle mass index(ASMI) was analyzed; the differences in CALLY index and CTI under different traditional Chinese medicine syndrome types were compared. Logistic regression analysis and receiver operating characteristic(ROC)curve were used to evaluate the application value of CALLY index and CTI in predicting the severity and prognosis of DN complicated with sarcopenia. Results:The CALLY index in the DN complicated with sarcopenia group was lower than that in the DN group,and the CTI was higher than that in the DN group,differences being significant(P<0.05). The CALLY index was positively correlated with the step count,grip strength,and ASMI(P<0.05),and the CTI was negatively correlated with the step count,grip strength,and ASMI (P<0.05) in the DN complicated with sarcopenia group. The CALLY index in the severe DN complicated with sarcopenia group was lower than that in the mild DN complicated with sarcopenia group,and the CTI was higher than that in the mild DN complicated with sarcopenia group,differences being significant(P<0.05). The CALLY index was negatively correlated with the severity of DN complicated with sarcopenia(P<0.05), and the CTI was positively correlated with the severity of DN complicated with sarcopenia(P<0.05). There were differences being found in the comparisons of CALLY index and CTI of the four traditional Chinese medicine syndrome types (P<0.05). The traditional Chinese medicine syndrome types with CALLY index from low to high,were spleen-kidney yang deficiency syndrome, stagnation of turbidity-toxin-stasis syndrome, yin-yang deficiency syndrome, and qi -yin deficiency syndrome,with significant differences between syndrome types(P<0.05). The traditional Chinese medicine syndrome types with CTI from high to low, were spleen-kidney yang deficiency syndrome,stagnation of turbidity-toxin-stasis syndrome,yin-yang deficiency syndrome,and qi-yin deficiency syndrome; the CTI of spleen-kidney yang deficiency syndrome,stagnation of turbidity-toxin-stasis syndrome,yin-yang deficiency syndrome was higher than that of qi-yin deficiency syndrome,respectively(P<0.05). The CTI of spleen-kidney yang deficiency syndrome was higher than that of stagnation of turbidity-toxin-stasis syndrome and yin-yang deficiency syndrome,respectively(P<0.05). The area under the ROC curve(AUC),sensitivity and specificity of CALLY index were superior to those of CTI. Conclusion: CALLY index and CTI have clinical application prospects in the severity, prognosis evaluation and microscopic differentiation of traditional Chinese medicine syndrome types in DN complicated with sarcopenia.
