Abstract:Objective:To observe the clinical effect of the combination use of Kaiqiao Liyan four-step needling and conventional swallowing training on post-stroke dysphagia. Methods: A total of 104 patients with post-stroke dysphagia admitted to Wenzhou TCM Hospital of Zhejiang Chinese Medical University from March 2022 to September 2023 were selected and divided into the control group and the study group according to the random number table method, with 52 cases in each group. Among them,1 case was lost to follow-up,2 cases were excluded in the control group, and 49 cases were finally included. In the study group, 1 case withdrew from the study voluntarily, 2 cases were excluded, and finally 49 cases were included. The control group was given swallowing training, and the study group was additionally given Kaiqiao Liyan four-step needling based on the swallowing training. Both groups were treated for four weeks continuously. After four weeks, the clinical efficacy in the two groups was evaluated; the scores of swallowing function, pharyngeal transit time, hyoid bone movement and incidence of complications were compared between the two groups. Results:The total effective rate was 93.88% (46/49) in the study group,higher than that of 79.59% (39/49) in the control group (P<0.05). After treatment, the scores of Standardized Swallowing Assessment (SSA) in the two groups were decreased when compared with those before treatment (P<0.05),and the SSA score in the study group was lower than that in the control group (P<0.05). After treatment, the scores of Videofluoroscopic Swallowing Study (VFSS) in the two groups were increased when compared with those before treatment( P<0.05),and the VFSS score in the study group was higher than that in the control group( P<0.05). After treatment,the pharyngeal transit time in the two groups was shortened when compared with that before treatment (P< 0.05),and the pharyngeal transit time in the study group was shorter than that in the control group (P<0.05). After treatment,the hyoid bone movement in the two groups was prolonged when compared with that before treatment (P< 0.05), and the hyoid bone movement in the study group was longer than that in the control group (P<0.05). The incidence of aspiration and aspiration pneumonia in the study group was lower than that in the control group (P<0.05); there was no significant difference being found in the comparison of the incidence of bronchospasm between the two groups( P>0.05). Conclusion:The combination use of Kaiqiao Liyan four-step needling and conventional swallowing training has a significant curative effect on post-stroke dysphagia, which can improve the swallowing function of patients,and decrease the incidence of dysphagia-related complications of aspiration and aspiration pneumonia.
