Abstract： Objective： To observe the clinical efficacy of Huoxue Tongluo Fumigation-Wash Prescription combined with conventional western medicine for diabetic foot （Wagner grade 0 to 1） with blood vessel obstruction syndrome. Methods： A total of 88 patients with diabetic foot （Wagner grade 0 to 1） with blood vessel obstruction syndrome admitted to the Endocrinology Department of Lishui People's Hospital from January 2022 to June 2024 were enrolled and divided into the control group and the treatment group using the random number table method， with 44 cases in each group. The control group received conventional western medicine treatment，while the treatment group received Huoxue Tongluo Fumigation-Wash Prescription in addition to conventional treatment. Both groups were treated for four weeks continuously. The clinical efficacy was evaluated， changes in traditional Chinese medicine syndrome scores，pain severity [Visual Analogue Scale （VAS） score]，toe skin temperature，Ankle Brachial Index （ABI），and nerve conduction velocity [motor nerve conduction velocity （MNCV） and sensory nerve conduction velocity （SNCV） of the common peroneal nerve and median nerve]， and hemodynamic indices of the dorsalis pedis artery [lumen diameter（ D），blood flow volume（ Vol），and peak velocity（ Vmax）] were compared between the two groups before and after treatment. Results：After four weeks of treatment，the total effective rate was 90.70% （39/43） in the treatment group， higher than that of 74.42% （32/43） in the control group （P＜0.05）. Traditional Chinese medicine syndrome scores and VAS scores decreased in both groups compared with those before treatment （P＜0.05），with the treatment group showing lower scores than the control group （P＜0.05）. Toe skin temperature and ABI increased in both groups compared with those before treatment （P＜0.05）， with the treatment group showing higher values than the control group （P＜0.05）. The MNCV and SNCV of the common peroneal and median nerves increased in both groups compared with those before treatment （P＜0.05）， with the treatment group showing higher values than the control group （P＜ 0.05）. The D and Vol of the dorsalis pedis artery increased，and the Vmax of the dorsalis pedis artery decreased in both groups compared with those before treatment （P＜0.05），with the treatment group showing greater D and Vol and lower Vmax than the control group （P＜0.05）. Conclusion：Huoxue Tongluo Fumigation-Wash Prescription combined with conventional western medicine is effective for diabetic foot （Wagner grade 0 to 1） with blood vessel obstruction syndrome. It can alleviate clinical symptoms， reduce pain， and increase toe skin temperature. Its mechanism may involve improving nerve conduction velocity and hemodynamics of the dorsalis pedis artery.
