Abstract： Objective： To observe the effects of Wenjing Tongluo Powder combined with Alprostadil on wound microvessel density and intermittent claudication in patients with diabetic foot （DF）. Methods： A total of 80 DF patients admitted to Jinhua Hospital of Traditional Chinese Medicine from February 2022 to February 2024 were retrospectively selected and divided into the Alprostadil group （39 cases） and the combination group （41 cases） based on clinical treatment methods. The Alprostadil group received Alprostadil alone，while the combination group received Wenjing Tongluo Powder combined with Alprostadil. The clinical efficacy， symptom scores， blood glucose indicator levels [fasting blood glucose （FBG），2-hour postprandial blood glucose （P2hBG），glycated hemoglobin （HbA1c）]， wound microvessel density and dorsalis pedis artery blood flow velocity， and levels of serum inflammatory factor indicators [high-sensitivity C-reactive protein （hs-CRP），tumor necrosis factor-α （TNF-α），interleukin-6 （IL-6）] were compared between the two groups. Results：After treatment，the total effective rate was 95.12% （39/41） in the combination group and 76.92% （30/39） in the Alprostadil group， with significant difference between the two groups （P＜0.05）. After treatment，the symptom scores for intermittent claudication，sensation of pain，sensation of numbness，and sensation of cold and heaviness decreased in both groups compared to before treatment （P＜0.05），and the above scores in the combination group were lower than those in the Alprostadil group （P＜0.05）. After treatment， the levels of FBG，P2hBG，and HbA1c decreased in both groups compared to before treatment （P＜0.05），and the levels of the above three indicators in the combination group were lower than those in the Alprostadil group （P＜0.05）. After treatment， wound microvessel density and dorsalis pedis artery blood flow velocity increased in both groups compared to before treatment （P＜0.05）， and the levels of the above two indicators in the combination group were higher than those in the Alprostadil group （P＜0.05）. After treatment， the levels of hs-CRP， TNF-α， and IL-6 decreased in both groups compared to before treatment （P＜0.05），and the levels of the above three indicators in the combination group were lower than those in the Alprostadil group （P＜0.05）. Conclusion：The combination of Wenjing Tongluo Powder and Alprostadil has significant therapeutic efficacy in the treatment of DF patients. It can increase the wound microvessel density， promote the foot blood circulation， and reduce the body's inflammatory responses， improve the symptoms of intermittent claudication.
