Introduction to LIU Shulin's Clinical Experience in Stage-Specific Treatment of Diabetic Foot Based on the Theory of "Warming Therapy is Not the Only One Choice for Activating Yang"
Abstract:Professor LIU Shulin, integrating YE Tianshi's theory of "warming therapy is not the only one choice for activating yang" with decades of clinical experience, proposes that the pathogenesis of diabetic foot is rooted in yang deficiency of the spleen and kidney, with damp, heat, blood stasis, and toxin blockage as the secondary manifestations, belonging to a syndrome of deficiency in root and excess in manifestation. In clinical practice, he emphasizes restoring the pathways for yang qi distribution and adopts a stage-specific treatment approach according to the pre-ulceration, ulceration, and recovery stages. In the pre-ulceration stage, the focus is on warming and unblocking, with treatments aimed at warming and tonifying the spleen and kidney, dispelling cold and removing dampness, and activating blood circulation to unblock collaterals. In the ulceration stage, the priority is eliminating pathogens and reinforcing healthy qi,by clearing heat and draining dampness, resolving stasis and removing toxins to remove necrosis and promote tissue regeneration, while also fortifying the spleen and warming the kidney. During the recovery stage,he advocates for simultaneous unblocking and tonifying, through supplementing qi and blood, warming and tonifying the spleen and kidney, while also dispersing residual pathogens, regulating qi and blood flow, and unblocking the meridians and collaterals.
