    目的:观察柴芩消痈饮对肝经蕴热型肉芽肿性小叶性乳腺炎(GLM) 患者乳腺组织中NF-κB、 PPAR-γ和巨噬细胞的影响,探讨柴芩消痈饮治疗肝经蕴热型肿块期GLM的作用机制。方法:于治疗前后穿刺 或手术时取肝经蕴热型肿块期6例GLM患者乳腺病灶组织标本作为疾病组及治疗组,另匹配6例非炎症非恶性 肿瘤患者的健康乳腺组织作为健康对照组。采用HE染色观察乳腺组织病理形态;采用免疫组织化学法观察 CD68标记的巨噬细胞表达情况;采用免疫荧光双染色法观察乳腺组织内M1型巨噬细胞(F4/80和iNOS) 和M2 型巨噬细胞(F4/80和CD206) 细胞标记;采用Western Blot方法检测各组乳腺组织内相关炎症因子肿瘤坏死因 子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6) 的表达,测定PPAR-γ/NK-κB信号通路上 相关蛋白的变化;检测M1细胞特异性表达蛋白iNOS、COX2以及M2细胞特异性表达蛋白IL-4、IL-10的表达。 结果:柴芩消痈饮可减轻GLM病变组织中炎症病理状态,降低病变组织中TNF-α、IL-6、IL-1β炎症因子的表 达,增加PPAR-γ蛋白、减少NF-κB p65蛋白的表达;可显著降低GLM病变组织中巨噬细胞的含量;结果显示 GLM肿块期病变组织中存在高表达M1型巨噬细胞代表分子iNOS和COX2,低表达M2型巨噬细胞标志分子 IL-4和IL-10;经柴芩消痈饮干预后能够降低乳腺组织中M1型巨噬细胞标志物iNOS,增加M2型巨噬细胞标志 物CD206 的分布和表达。结论:柴芩消痈饮能够减轻局部炎症因子表达、降低巨噬细胞表达、通过抑制 PPAR-γ/NK-κB信号通路促进巨噬细胞由M1向M2极化发挥对GLM治疗作用。

    Abstract:

    Abstract: Objective: This study observed the effects of Chaiqin Xiaoyong Decoction on the PPAR-γ/NF-κB signaling pathway and macrophages in breast tissue of patients with granulomatous lobular mastitis (GLM) of heat accumulation in the liver meridian type,and explored the mechanism by which Chaiqin Xiaoyong Decoction treats GLM during the mass-forming stage of this type. Methods:Before and after treatment,breast-lesion tissue specimens were obtained by biopsy or surgery from six patients with mass-stage GLM of heat accumulation in the liver meridian type; these samples served as the disease group and treatment group. Six healthy breast tissues from patients without inflammation or malignancy were matched as healthy controls. The HE staining was used to observe histopathological morphology. Immunohistochemistry was performed to evaluate CD68-labelled macrophages. Double immunofluorescence was employed to identify M1 macrophages (F4/80 and iNOS) and M2 macrophages (F4/80 and CD206) within the tissue. Western blot was applied to measure the expression of inflammatory cytokines including tumor necrosis factor- α (TNF-α),interleukin (IL)-1β and IL-6;changes in PPAR-γ/NF-κB pathway-related proteins were detected, and M1-specific markers (iNOS and COX2) as well as M2-specific markers (IL-4 and IL-10) were quantified. Results: Chaiqin Xiaoyong Decoction alleviated inflammatory pathology, reduced TNF- α, IL-6 and IL-1β expression,up-regulated expression of PPAR-γ protein and down-regulated expression of NF-κB p65 protein in GLM lesions,and significantly decreased total macrophage infiltration. GLM tissue in the mass stage showed high expression of M1 markers iNOS and COX2 and low expression of M2 markers IL-4 and IL-10. After intervention with Chaiqin Xiaoyong Decoction,M1 marker iNOS expression was decreased while the distribution and expression of the M2 marker CD206 were increased. Conclusion: Chaiqin Xiaoyong Decoction can alleviate local inflammatory factor expression, reduce macrophage infiltration,and promote the switch of macrophages from the M1 to the M2 phenotype by regulating the PPAR-γ/NF-κB signaling pathway,thereby exerting its therapeutic effect on GLM.
