From Sinew Channels to Myofascial Chains:A Clinical Study on Manipulative Massage Combined with Shock Wave Therapy for Plantar Fasciitis-Related Heel Pain
Abstract: Objective: To observe the clinical effect of the combination use of manipulative massage and extracorporeal shock wave therapy (ESWT) on plantar fasciitis-related heel pain based on myofascial chain theory. Methods: A total of 60 plantar fasciitis-related heel pain patients admitted to Shunde Hospital of Guangzhou University of Chinese Medicine from January 2023 to December 2024 were selected and divided into the observation group and the control group according to the random number table method, with 30 cases in each group. The control group was treated with conventional ESWT,and the observation group was treated with manipulative massage combined with ESWT based on myofascial chain theory. The scores of Visual Analogue Scale (VAS) for pain, Maryland foot scores,plantar fascia thickness,and C-reactive protein( CRP) levels were compared between the two groups,and the clinical efficacy was evaluated. Results:After treatment,the total effective rate was 100% (30/30) in the observation group and 90.00% (27/30) in the control group, the difference being significant (P<0.05). After treatment one and three months of follow-up,the VAS scores in the two groups were decreased compared with those before treatment (P< 0.05),and the VAS score in the observation group was lower than that in the control group at the same time point (P< 0.05). After treatment one and three months of follow-up, the Maryland foot scores in the two groups were increased compared with those before treatment (P<0.05),and the Maryland foot score in the observation group was higher than that in the control group at the same time point (P<0.05). After treatment, the plantar fascia thickness in the two groups was decreased compared with that before treatment (P<0.05), and the plantar fascia thickness in the observation group was less than that in the control group (P<0.05). After treatment,the CRP levels in the two groups were reduced compared with those before treatment (P<0.05),and the CRP level in the observation group was lower than that in the control group (P<0.05). Conclusion:The combination use of manipulative massage and ESWT based on myofascial chain theory can effectively control the inflammatory responses of plantar fasciitis-related heel pain patients,mitigate pain and improve foot function,and reduce the thickness of plantar fascia.
