Abstract:Traditional Chinese medicine holds that precancerous lesions of gastric cancer (PLGC) classified as a syndrome of deficiency in the root and excess in the branch. The deficiency in origin is primarily weakness of the spleen and stomach,while the excess in the branch manifests as damp-heat,qi stagnation,and blood stasis. Professor ZHOU Fusheng proposes that the fundamental pathogenesis of PLGC is weakness of the spleen and stomach, with common syndrome types including damp-heat of the spleen and stomach, spleen deficiency with phlegm-damp, spleen deficiency with liver stagnation,and qi and yin deficiency. Furthermore,by integrating the five common constitutions frequently observed in the Lingnan region, namely, qi deficiency, yin deficiency, blood stasis, liver stagnation, and damp-heat, he addresses the root pathogenesis from the view of "disease-syndrome-constitution" integration, modifying treatments according to the individual condition. Clinically, he emphasizes both macro-level differentiation (symptoms, tongue manifestation, pulse condition) and micro-level differentiation (gastroscopy, pathology findings), achieving mutual verification and complementarity between the two. The therapeutic principle focuses on strengthening the spleen and supplementing qi as the foundation, simultaneously addressing clearing and transforming damp-heat, invigorating the spleen, softening the liver, and with attention to balancing dynamic and static qualities. These are supplemented with fragrant herbs effective in eliminating dampness, activating blood and resolving stasis, and clearing heat and removing toxin, aiming to achieve simultaneous treatment of both root and branch,and harmonization of deficiency and excess syndromes.
