Analysis on Features of DING Ganren Grandfather-Grandson Lineage of Shanghai School of Traditional Chinese Medicine in Treating Strangury with Turbid Discharge by Syndrome Differentiation
Abstract: DING Ganren is a famous Chinese medicine physician in modern times, one of the four masters of Menghe medical school, and is famous for his command of internal medicine in Shanghai. There are five pieces of prescription notes for strangury with turbid discharge collected by DING Ganren and his eldest grandson DING Jiwan in Shanghai Museum of Traditional Chinese Medicine. Taking the collection of prescription notes as an example,this paper discusses and analyzes the theory, method, prescription and medicine of DING in treating strangury with turbid discharge,and summarizes the clinical features. According to DING's theory,the pathogenesis of strangury with turbid discharge is characterized by dampness in the liver channel,heat in the heart channel,and stasis in the urethra as the manifestation, and kidney yin deficiency as the root. In treatment, the etiology must be carefully analyzed, and the excess and deficiency should be assessed. The treatment of strangury with turbid discharge is divided into three stages by syndrome differentiation. At acute stage, the excess syndrome is dominant, and it should clear damp-heat from the lower energizer and promote blood circulation to relieve strangury; at chronic protracted stage, treatment should be based on either excess syndrome or complex syndrome of excess-deficiency by nourishing yin and clearing heat; at convalescent recovery stage, deficiency syndrome is dominant, it should tonify kidney and strengthen spleen, and promote diuresis to eliminate dampness. DING's approach to treating strangury with turbid discharge transcends the traditional "five types of strangury and two types of turbidity" classification system. Instead,they adopt a stage-specific treatment therapy based on disease progression and the excess-deficiency balance,innovating the method of nourishing yin and clearing heat for complex syndrome.
