Abstract: Objective: To observe the effects of Qingdai Jiedu Decoction combined with conventional western medicine on vaginal microenvironment, viral load, and HPV E6/E7 mRNA in patients with cervical high-risk human papillomavirus (HR-HPV) infection. Methods: A total of 96 patients infected with cervical HR-HPV who were admitted to the outpatient department of Anyang District Hospital from January to December 2024 were selected and divided into the control group and the observation group according to the random number table method, with 48 cases in each group. Among them, 2 cases dropped out in the control group and 46 cases were finally included. In the observation group, 3 cases dropped out and 45 cases were finally included. The control group was treated with conventional western medicine, and the observation group was additionally treated with Qingdai Jiedu Decoction based on the treatment in the control group. Both groups were treated for three months continuously. The clinical efficacy, traditional Chinese medicine syndrome scores, HPV viral load, vaginal microenvironment, and HPV E6/E7 mRNA expression levels were compared between the two groups. Results:The total effective rate was 88.89% (40/45) in the observation group, which was higher than that of 67.39%( 31/46) in the control group( P<0.05). After treatment, the sub-item scores and total scores of traditional Chinese medicine syndromes in the two groups were decreased when compared with those before treatment (P<0.05),and the above scores in the observation group were lower than those in the control group (P<0.05). After treatment, the HPV viral load in the two groups was reduced when compared with that before treatment (P<0.05), and the HPV viral load in the observation group was lower than that in the control group (P<0.05). After treatment, the proportion of vaginal cleanliness (Ⅰ-Ⅱ),the proportion of vaginal pH (3.8- 4.5),the proportion of leukocyte esterase negative, the proportion of catalase negative, and the proportion of negative acetylglucosaminidase in the two groups were significantly increased when compared with those before treatment (P< 0.05), and proportion of vaginal cleanliness (Ⅰ-Ⅱ), the proportion of vaginal pH (3.8-4.5), the proportion of leukocyte esterase negative, the proportion of catalase negative, and the proportion of negative acetylglucosaminidase in the observation group were higher than those in the control group (P<0.05). After treatment, the expression levels of HPV E6/E7 mRNA in the two groups were down-regulated when compared with those before treatment (P<0.05),and the expression level of HPV E6/E7 mRNA in the observation group were lower than those in the control group (P< 0.05). Conclusion: Qingdai Jiedu Decoction combined with conventional western medicine is helpful to promote the negative conversion of cervical HR-HPV infection in patients, reduce viral load, improve vaginal microenvironment, and reduce the risk of cervical cancer.
