Cross-Sectional Study on Basic Characteristics and Traditional Chinese Medicine Constitution Distribution in 200 Pre-Chronic Obstructive Pulmonary Disease Cases
Abstract:Objective:To analyze the basic characteristics and traditional Chinese medicine (TCM) constitution distribution in a pre-chronic obstructive pulmonary disease (pre-COPD) population, and to provide evidence-based medical basis for the identification and integrated traditional Chinese and western medicine prevention and treatment of pre-COPD. Methods: Using a cross-sectional study design, data were collected from pre-COPD individuals diagnosed between July 2024 and January 2025 at Guangdong Provincial Hospital of Chinese Medicine, Guangzhou Kindness Health Care Medical Center, Dongguan Hospital of Integrated Traditional Chinese and Western Medicine, and Dongguan Hengli Community Health Service Center. The collected data included general information,COPD risk factors, respiratory symptoms, TCM four diagnostic information, and TCM constitutions. Descriptive statistical analysis and binary Logistic regression were used to analyze the basic characteristics and TCM constitution distribution patterns of the population. Results: A total of 200 pre-COPD individuals were surveyed. Analysis revealed that the small airway dysfunction (SAD) subtype was the most common (103 cases,51.50%),followed by the preserved ratio impaired spirometry (PRISm) combined with SAD group (31 cases, 15.50%), and the chronic bronchitis (CB) complicated with SAD group (18 cases,9.00%). The main respiratory symptoms were cough (94 cases,47.00%) and expectoration( 88 cases,44.00%). Occupational exposure history and smoking history were risk factors for the presence of respiratory symptoms in the pre-COPD population (P<0.05). Common TCM four diagnostic information included occasional cough (82 cases, 41.00%), shortness of breath (75 cases, 37.50%), greasy coating (144 cases, 72.00%),white coating( 137 cases,68.50%),and thready pulse( 53 cases,26.50%). Regarding TCM constitutions, biased constitutions were common (126 cases, 63.00%), with yang deficiency constitution being the most common (57 cases,28.50%). Conclusion:The distribution of pre-COPD population is predominantly characterized by the SAD subtype, with most individuals exhibiting respiratory symptoms, primarily cough and expectoration. Smoking history and occupational exposure history are risk factors for developing respiratory symptoms. Common tongue manifestations are greasy coating and white coating;a common pulse condition is the thready pulse. Among TCM biased constitutions,yang deficiency constitution is the most common.
