Abstract: Objective: To observe the clinical efficacy of Zijing Zhitong Cataplasm in the treatment of knee osteoarthritis (KOA) of dampness-heat blockage type. Methods: A total of 100 patients with KOA admitted to Guangzhou Hospital of Integrated Traditional and West Medicine from July 2023 to April 2025 were enrolled and divided into the control group and the observation group using the random number table method,with 50 cases in each group. The control group was treated with Celecoxib Capsules,while the observation group received Zijing Zhitong Cataplasm in addition to the control group's treatment. The treatment course for both groups was four weeks continuously. The changes in Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score,interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), traditional Chinese medicine syndrome scores, walking speed before and after treatment, as well as clinical efficacy and incidence of adverse reactions were compared between the two groups. Results:After treatment,the total effective rate was 94.00%( 47/50) in the observation group and 82.00%( 41/50) in the control group, with a statistically significant difference between the two groups (P<0.05). After treatment, the WOMAC scores of both groups decreased compared with those before treatment (P<0.05), and the score of the observation group was lower than that of the control group (P<0.05). After treatment,the levels of TNF-α and IL-6 in both groups decreased compared with those before treatment (P<0.05), and the levels of TNF-α and IL-6 in the observation group were lower than those in the control group (P<0.05). After treatment, the traditional Chinese medicine syndrome scores for joint swelling and pain,skin temperature around the joint,limited flexion and extension of the joint, and tongue manifestation and pulse condition in both groups decreased compared with those before treatment (P<0.05), and all scores in the observation group were lower than those in the control group (P<0.05). After treatment,the walking speed of both groups increased compared with that before treatment (P<0.05),and the walking speed of the observation group was greater than that of the control group (P<0.05). During treatment, the incidence of adverse reactions was 10.00% (5/50) in the observation group and 12.00% (6/50) in the control group, with no statistically significant difference between the two groups (P>0.05). Conclusion:Zijing Zhitong Cataplasm in the treatment of KOA of dampness-heat blockage type can improve clinical efficacy, effectively reduce systemic inflammatory response, alleviate symptoms such as knee swelling, pain, and limited movement, and improve knee function and walking speed.
