Abstract: Constipation is a highly prevalent complication of stroke that significantly impacts patients' quality of life and rehabilitation progress. The sweat pores and collaterals are fundamental components of the human body,serving as pathways for the movement of qi and fluids. Based on the "sweat pores, qi-fluids, and collaterals" theory, this paper proposes a stage-specific treatment approach for post-stroke constipation (PSC) . It suggests that the pathogenesis in the acute stage is characterized by stagnation and obstruction of sweat pores,impaired diffusion of qi and fluids,and obstruction of the collaterals. In the recovery stage, the pathogenesis evolves to blockage of the sweat pores and stagnation of the collaterals,with impairment of qi and fluids,and retention of phlegm and blood stasis. In the sequela stage,the pathogenesis involves deficiency and stagnation of qi and fluids,damage to the sweat pores and impairment of the collaterals, and latent retention of hidden pathogens. Accordingly, a stage-specific treatment approach is proposed: For the acute stage, which is primarily characterized by excess pathogens, the treatment should focus on unblocking the fu-organs and opening the sweat pores, as well as unblocking the collaterals to eliminate pathogens, using modified Da Chengqi Decoction. For the recovery stage, which is characterized by deficiency interweaved with excess syndrome, the treatment should combine unblocking and tonifying methods to unblock the sweat pores and promote the free flow of the collaterals,using modified Buyang Huanwu Decoction combined with Maziren Pills. For the sequela stage,which is primarily characterized by deficiency syndrome,the treatment should focus on replenishing qi and fluids of the sweat pores and enriching essence, as well as unblocking the collaterals and expelling pathogens, using modified Bazhen Decoction combined with Jichuan Decoction. This aims to provide a reference for the syndrome differentiation and treatment of PSC in traditional Chinese medicine.
