Effect of LIN Dingkun's Muscle-Strengthening and Bone-Enhancing Exercise Regimen on Rehabilitation in Elderly Patients After Internal Fixation for Intertrochanteric Femoral Fracture
Abstract: Objective: To observe the effect of LIN Dingkun's muscle-strengthening and bone-enhancing exercise regimen on functional recovery in elderly patients after internal fixation for intertrochanteric femoral fracture. Methods: A total of 120 elderly patients who underwent internal fixation for intertrochanteric femoral fracture at Guangdong Provincial Hospital of Chinese Medicine from January 2020 to August 2021 were selected as study subjects. They were randomly divided into the exercise group and the control group using the random number table method,with 60 cases in each group. The control group received postoperative routine interventions including nutritional support and pain management, and began conventional rehabilitation exercises on the day of surgery. The exercise group received the same postoperative routine interventions and began practicing LIN Dingkun's muscle-strengthening and boneenhancing exercise regimen on the day of surgery. Both groups were observed for 12 weeks continuously. Muscle strength of the quadriceps,iliopsoas,and tibialis anterior,upper limb grip strength,and vital capacity were measured,frailty status was assessed on postoperative day 1,day 3,week 1,week 4,and week 12. Balance ability and sit-to-stand time were tested at postoperative week 1,week 4,and week 12. Results:With the extension of postoperative recovery time,both groups showed a gradual increasing trend in the muscle strength of the quadriceps,iliopsoas,and tibialis anterior,as well as in upper limb grip strength and vital capacity (P<0.05). The muscle strength of the quadriceps, iliopsoas,and tibialis anterior,as well as the upper limb grip strength in both groups were higher at postoperative day 3, week 1, week 4, and week 12 compared to the previous time point (P<0.05). The muscle strength of the quadriceps, iliopsoas, and tibialis anterior in the exercise group was higher than that in the control group at postoperative week 1,week 4,and week 12 (P<0.05). The upper limb grip strength in the exercise group was higher than that in the control group at postoperative week 4 and week 12 (P<0.05). In the control group, vital capacity increased at postoperative day 3, week 4, and week 12 compared to the previous time point (P<0.05). The vital capacity in the exercise group was greater than that in the control group at postoperative week 1, week 4, and week 12( P<0.05). With the extension of postoperative recovery time,the balance test time in both groups showed a gradual increasing trend (P<0.05), while the sit-to-stand time showed a gradual decreasing trend (P<0.05). The balance test time in both groups was longer at postoperative week 4 and week 12 compared to the previous time point (P<0.05). The balance test time in the exercise group was longer than that in the control group at postoperative week 4 and week 12( P<0.05). The sit-to-stand time in both groups at postoperative week 12 was shorter than that at postoperative week 4( P<0.05). The sit-to-stand time in the exercise group was shorter than that in the control group at postoperative week 12( P<0.05). The frailty grade in the exercise group was superior to that in the control group at both postoperative week 4 and week 12 (P<0.05). Conclusion:The application of LIN Dingkun's muscle-strengthening and bone-enhancing exercise regimen for functional exercise in elderly patients after internal fixation for intertrochanteric femoral fracture can effectively improve their muscle strength,vital capacity,and balance ability,enhance their motor function,effectively prevent frailty,and promote postoperative fracture recovery.
