Clinical Study on Modified Wuzi Yanzong Decoction Combined with Western Medicine for Parkinson's Disease Complicated by Depression of Kidney Deficiency and Marrow Depletion with Liver Stagnation Type
Abstract: Objective: To observe the clinical efficacy of modified Wuzi Yanzong Decoction combined with western medicine in the treatment of Parkinson's disease complicated by depression (dPD) of kidney deficiency and marrow depletion with liver stagnation type, and to evaluate the effects of the combined regimen on oxidative stress markers,inflammatory factors,and T lymphocyte subsets.Methods:A total of 60 dPD patients of kidney deficiency and marrow depletion with liver stagnation type,who visited the Neurology and Rehabilitation Departments of Wenzhou TCM Hospital of Zhejiang Chinese Medical University from May to July 2024, were enrolled. They were randomly divided into the treatment group and the control group using the random number table method,with 30 cases in each group. The control group received Levodopa and Benserazide Hydrochloride Tablets and Paroxetine Hydrochloride Tablets.The treatment group received modified Wuzi Yanzong Decoction in addition to the control group's treatment. Both groups were treated for eight weeks.The scores of the Unified Parkinson's Disease Rating Scale Part Ⅲ( UPDRS-Ⅲ), Hamilton Depression Scale-24 items (HAMD-24), traditional Chinese medicine syndrome, Parkinson's Disease Questionnaire-39 items (PDQ-39),as well as the levels of superoxide dismutase (SOD),inflammatory factors [tumor necrosis factor-α (TNF-α),interleukin-6 (IL-6)],and T lymphocyte subset indicators (CD4+ , CD8+ , CD4+/CD8+ ratio) were compared before and after treatment between the two groups. Treatment safety was assessed. Results: After eight weeks of treatment,the scores of UPDRS-Ⅲ,HAMD-24,traditional Chinese medicine syndrome,and PDQ-39 in both groups decreased compared with those before treatment.The scores in the treatment group were lower than those in the control group,and the differences were statistically significant (P<0.05).The levels of SOD,CD4+, and the CD4+/CD8+ ratio increased in both groups compared with those before treatment, while the levels of TNF-α, IL-6,and CD8+ decreased,with statistically significant differences (P<0.05).The treatment group showed higher levels of SOD,CD4+,and CD4+/CD8+ ratio,and lower levels of TNF-α,IL-6,and CD8+ than the control group, with statistically significant differences (P<0.05).No serious drug-related adverse reactions were observed in either group. Blood routine tests, urine routine tests, liver function tests, kidney function tests, and electrocardiograms showed no abnormalities. Conclusion: Modified Wuzi Yanzong Decoction combined with western medicine can improve traditional Chinese medicine syndromes, depressive symptoms, motor function, and quality of life in dPD patients of kidney deficiency and marrow depletion with liver stagnation type. The mechanism may be related to alleviating oxidative stress damage,inhibiting inflammatory responses,and regulating immune balance.
