Abstract:Objective:To observe the clinical effect of Shenfu Injection assisting conventional western medicine on chronic heart failure (CHF) of yang-qi deficiency type and its effect on oxidative stress and iron metabolism indexes. Methods:A total of 63 CHF patients of yang-qi deficiency type who were treated in the Department of Critical Care Medicine of Zhongshan Hospital of Chinese Medicine Affiliated to Guangzhou University of Chinese Medicine from January to May 2024 were enrolled and randomly divided into the observation group (32 cases) and control group (31 cases) using random numbers generated by Matlab statistical software. The control group was treated with conventional western medicine until discharge. On this basis, the observation group was intravenously pumped with Shenfu Injection for 72 hours. The clinical efficacy, total length of stay, and liver and kidney function indicators at discharge, as well as the total score of traditional Chinese medicine syndromes, and score of Acute Physiology and Chronic Health Evaluation (APACHEⅡ) were compared. Critical care ultrasound monitoring indexes [left ventricular ejection fraction (LVEF), lung B-line scores], N-terminal pro-brain natriuretic peptide (NT-proBNP), serum superoxide dismutase (SOD),iron metabolism index were measured between the two groups. Results:The markedly effective rate was 56.67% (17/30) in the observation group and 36.67% (11/30) in the control group,there being no significant difference between the two groups (P>0.05). At discharge,the total scores of traditional Chinese medicine syndromes in the two groups were decreased when compared with those before treatment (P<0.05),and the total scores of traditional Chinese medicine syndromes in the observation group were lower than those in the control group (P< 0.05). The values of LVEF in the two groups were increased when compared with those before treatment (P<0.05), and the B-line scores were decreased when compared with those before treatment (P<0.05);the LVEF value in the observation group was higher than that in the control group (P<0.05),and the B-line scores were lower than those in the control group( P<0.05). The levels of NT-proBNP in the two groups were reduced when compared with those before treatment (P<0.05);the level of NT-proBNP in the observation group was lower than that in the control group,there being no significant difference between the two groups (P>0.05). The APACHE Ⅱ scores in the two groups were decreased when compared with those before treatment (P<0.05),and the APACHEⅡscores in the observation group were lower than those in the control group (P<0.05). The serum SOD levels in the two groups were increased when compared with those before treatment (P<0.05),and the serum SOD level in the observation group was higher than that in the control group (P<0.05). The levels of ferritin and unsaturated iron binding capacity in the two groups were decreased when compared with those before treatment (P<0.05),and the levels of transferrin,iron ion and total iron binding capacity were increased when compared with those before treatment (P<0.05); the level of ferritin in the observation group was lower than that in the control group (P<0.05),and the levels of transferrin,iron ion and total iron binding capacity were higher than those in the control group (P<0.05). There was no significant difference being found in the comparison of unsaturated iron binding capacity between the two groups (P>0.05). There were no significant differences being found in the comparisons of total length of stay and the levels of alanine aminotransferase and creatinine at discharge between the two groups (P>0.05). Conclusion: Shenfu Injection assisting conventional western medicine can effectively inhibit oxidative stress and regulate iron metabolism of CHF patients of yang-qi deficiency type, enhance myocardial contractility and alleviate pulmonary congestion, so as to improve the cardiac function and relieve the clinical symptoms,without increasing the risk of medication.
